The Job

Arrowhead Regional Medical Center (ARMC) is recruiting for a Patient Navigator who assists and support cancer patients in the Patient Navigation Program by facilitating timely medical care to avoid delays in treatment.

Duties include, but are not limited to, the following:
  • Assists the Patient Navigation Program Coordinator with facilitation of cancer patients' referrals to specialty clinics, treatment, follow-up appointments, and diagnostic scheduling.
  • Consults with physicians and other members of the care delivery team on the patient's treatment plan; refers to appropriate physician or department program as needed.
  • Promotes and provides patient, family, staff and community education and awareness related to the Patient Navigation Program, cancer treatments, and services at the Medical Center.
  • Provides emotional support to the patient and families when needed; and must be available for on-call duty as needed.
  • Tracks the patient outcomes and relevant data using manual or automated systems; collaborates with the tumor registrars on statistical reporting.
  • Participates in community outreach via Cancer Screening events held at the Medical Center.
  • Facilitates paperwork and guides patients in the application process for financial coverage.
  • Serves as a chaperone for physicians when needed in the treatment or examination of a patient.

ARROWHEAD REGIONAL MEDICAL CENTER

Located on a beautiful 70-acre campus in Colton, California (50 miles east of Los Angeles), Arrowhead Regional Medical Center (ARMC) is a state-of-the-art, public/nonprofit, 456-bed, university affiliated teaching hospital licensed by the State of California Department of Public Health and accredited by The Joint Commission. The hospital houses a regional burn center serving four counties (San Bernardino, Riverside, Mono and Inyo), a comprehensive stroke center, level I trauma center and a freestanding in-patient behavioral health center. Additionally, ARMC operates five community-based, primary care clinics and over 40 specialty care outpatient services. ARMC is the primary teaching hospital for the adjacently located California University of Science and Medicine (CUSM), the Inland Empire's newest medical school.

CONDITIONS OF EMPLOYMENT

Pre-Employment Process: Applicants selected for these positions must pass a background investigation which includes verification of employment history and education as well as a physical exam and drug screening.

Travel: Travel throughout the County may be required. At the time of hire, a valid California driver license and proof of automobile liability insurance must be produced and maintained for the individual providing the transportation.

Certification: Positions within Clinical Operations may require a Healthcare Provider level Basic Life Support (BLS) certification.

Sponsorship: San Bernardino County is not able to consider candidates who will require visa sponsorship at the time of application or in the future.

Minimum Requirements

Applicants must meet one of the options listed below:

Option 1

Experience: One (1) year of experience providing healthcare, mental health, social or human services. Experience must include one of the following: performing needs assessments, conducting interviews, referring clients to services or performing crisis interventions.

Option 2

Experience: One (1) year of paid or volunteer experience in a mental health, substance use, social, or human services program within a San Bernardino County program or a San Bernardino County auxiliary service provider.

Option 3

Certification: Successful completion of a Community Health Workers (CHW) program resulting in a certificate. A legible copy of the CHW certificate must be attached to the online application or summitted by the filing deadline.

Desired Qualifications

The ideal candidate will possess the ability to communicate effectively in person, in writing, and on the telephone, be organized with attention to detail, be adaptable, and possess critical thinking skills. Furthermore, the ideal candidate will be culturally sensitive to the population being served, possess basic understanding of medical terminology ,and general knowledge of Microsoft Office Suite (Outlook, Word, Excel, and PowerPoint).

English/Spanish bilingual skills are highly desired. For consideration of bilingual assessments, applicants must indicate bilingual ability on the application. Employees may receive additional compensation for utilizing such skills when allocated to a bilingual position, as approved by the department. Bilingual testing may be required.
