Job Description

DESCRIPTION

The City of Long Beach, Department of Health and Human Services is recruiting an Outreach Worker II in the Community Health Bureau, Clinical Services Division. Reporting to a Public Health Nurse III, this position provides critical outreach services for the Tuberculosis Clinic and is an integral part of the disease prevention and care team.

EXAMPLES OF DUTIES

  • Offers information, education, referrals, and linkage assistance to the community;
  • Assists with program service goals, objectives and mandates;
  • Conducts, interviews, manages, and follows up on tuberculosis cases with hospitals, doctors, and patients;
  • Tracks, refers, and maintains regular contact with clients to ensure care and follow-up;
  • Arranges for meeting times and locations that meet the needs of both the patient and DOT worker;
  • Maintains patient confidentiality with all encounters;
  • Assists patients with completion of regimen;
  • Delivers prescribed medication to patients;
  • Observes the patient to ensure that correct pills are being taken by the correct patient on the correct day;
  • Observes for possible signs of non-adherence (i.e. not swallowing medication);
  • Asks patients about symptoms of adverse drug reactions to medication and reports potential reactions to the case manager and/or supervisor;
  • Documents medications that were taken and other relevant findings to complete adequate documentation for the services/observations rendered;
  • Reinforces counseling and education messages provided by the case manager during all encounters;
  • Participates in the needs evaluation and distribution of DOT incentives and enablers;
  • Attends community events and fairs;
  • Provides information and referrals;
  • May provide translation assistance to non-English-speaking clients;
  • May need to assist with rooming tuberculosis patients when in the clinic;
  • May transport clients to the clinic;
  • Performs data entry;
  • Performs other related duties as required.


REQUIREMENTS TO FILE

EDUCATION
  • High School diploma, GED or equivalent

EXPERIENCE
  • Two (2) years of professional full-time equivalent experience working in outreach programs. (Academic degrees in Public Health, Health Sciences, or a closely related field may be substituted for required experience on a year-for-year basis.

ADDITIONAL REQUIREMENT
  • A valid motor vehicle license.

DESIRABLE QUALIFICATION:
  • Willing and able to work evenings, nights, weekends, holidays, and on-call during emergencies as needed.
