There are currently four (4) vacancies located in Concord, CA. You may be required to occasionally travel to different locations throughout Contra Costa County.

Community Health Worker Specialists perform complex clinical support and outreach duties related to preventative and supportive health care services in a variety of public health programs and clinics; provides a variety of health program services to clients; provides lead direction to Community Health Workers and/or independently conducts a variety of community outreach activities; and performs related work as required.

Contra Costa County Health Services mission is to care for and improve the health of all people in Contra Costa County with special attention to those who are most vulnerable to health problems.

What you will typically be responsible for:
  • Providing lead direction to Community Health Workers which may include the assignment of tasks, the distribution of client cases, training of workers, reviewing their work and providing input on their performance
  • Independently conducting initial client and family needs evaluations in accordance with clearly defined protocols and provides health information and guidance to clients and family members accordingly
  • Interviewing clients and their families in the clinic and/or at home to develop a social/health care needs plan in accordance with protocols or in consultation with the supervisory staff or Public Health Nurse, professional staff, or nursing staff
  • Providing health information to clients on issues such as infant care and clothing, contraceptive methods, dental health, nutrition, pre-natal care, etc
  • Providing orientation to clients regarding available program services
  • Initiating contact with health and social service providers
  • Maintaining stock of educational materials

The eligible list established from this recruitment may remain in effect for six (6) months.

Minimum Qualifications

License Required: Possession of a valid California Motor Vehicle Operator's License. Out of state valid Motor Vehicle Operator's License will be accepted during the application process.

Education: Possession of a high school diploma, G.E.D. equivalency or a high school proficiency certificate.

Experience: Three (3) years of full-time or its equivalent experience having direct responsibility or participation in providing community outreach, client education, training and support services in a community program that promotes health and health education.

Substitution for experience: Completion of 60 semester or 90 quarter units of college coursework must have included 6 semester or 9 quarter units in health related coursework, or completion of a Community Health Worker Associates Degree, or Community Health Worker Certification Program from an accredited college or university may be substituted for twelve (12) months of the required experience.

Desirable Qualifications:
  • Ability to understand and speak Spanish


Selection Process

Application Filing and Evaluation: All applicants will be required to complete a supplemental questionnaire at the time of application. Applications will be evaluated to determine which candidates will move forward in the next phase of the recruitment process.Oral Interview: Candidates who possess the minimum qualifications will be invited to participate in an online assessment. The online assessment will measure candidates' competencies as they relate to the job and candidates must achieve an average passing score of 70% or higher on each of the competencies, as well as an overall passing score of 70% or higher. These competencies may include but are not limited to: Critical Thinking, Decision Making, Handling Stress, Self-Management and Building & Maintaining Relationships. (Weighted 100%)
The Oral Interview is tentatively scheduled to take place via computer (remotely) during the week of July 25, 2024.

The Oral Interview will be administered remotely using a computer or mobile device such as a tablet or smartphone. You will need access to a reliable internet connection to take the assessment, as well as a computer or mobile device with a camera.

The Human Resources Department may change the examination steps noted above in accordance with the Personnel Management Regulations and accepted selection practices.
