What you’ll do:

The position coordinates admissions and discharges to and from the inpatient setting. The Case Coordinator is responsible for working with the inpatient interdisciplinary treatment team in planning admissions and discharges. This position will also coordinate the guardianship process for patients, working collaboratively with the treatment teams and ensuring DMH Forensic Guidelines are followed. They will need to provide warm-hand offs in transition of services when patients discharge from NMPRC. The Case Coordinator will represent NMPRC with other agencies and partners to ensure needs of individuals served are met. Duties include, but are not limited to:

  • Provide person centered and trauma informed services when speaking with individuals in need.
  • Make follow-up calls or visits, as required.
  • Act as the primary contact for community providers and housing agencies regarding discharge planning.
  • Collaborate with inpatient social workers and other treatment team members in making discharge arrangements for clients.
  • Communicate clearly, timely, and effectively with the inpatient teams and community partners regarding admissions, discharges, guardianship packets, and forensic guidelines.
  • Maintain client records, following agency and state regulations.
  • Ensure all documentation is written in proficient and professional manner.
  • Educate inpatient treatment teams about available community resources and housing options.
  • Participate in discharge planning meetings with the inpatient treatment teams and community providers.
  • Assist the Director of Social Work and Director of Treatment Services in collecting data related to admissions and discharges.


Experience and Education Required:

  • Two years of experience in a similar role is preferred.
  • Individuals with degrees or experience in related fields, such as Psychology, Sociology, or Criminal Justice are encouraged to apply.
