Job Description

Role

The ideal candidate will have u nderstanding of federally qualified health centers and integrated care models , experience in using electronic health records including NextGen and be bilingual in Spanish.

Under general direction, responsible for the provision of behavioral health care to the Community Health Center of Yavapai (CHCY) patients.

Major Duties, Responsibilities

  • Provides comprehensive, patient focused behavioral health care to patients of the CHCY in an integrated model by collaborating with the patient’s primary care provider.
  • Behavioral health services including assessment, diagnosis, prescription of drug therapy, planning of treatment and evaluation of therapy. Treatment maybe in individual or in groups.
  • Refers patients to collaborating physicians, other specialists, and other medical facilities as needed. Provides patient education as needed.
  • Documents medical visit in the electronic medical records system.
  • Coordinates patient care with the clinic support staff.
  • Participates in the Quality Improvement Program, as requested.
  • Provides patient teaching as needed.
  • Participates in the Quality Improvement Program, as requested.
  • Responds to community emergencies with a public health consequence according to the Community Health Services Emergency Response Plan.
  • Performs other job-related duties as assigned.


Minimum Qualifications

Education:
Master’s Degree in Nursing and certification as a Psychiatric Nurse Practitioner or equivalent under the “grandfather clause”.

Experience:
Five years of Psychiatric Nurse Practitioner/Provider experience similar to the job responsibilities of this position.

Licensure or Certification:
Must possess a current State of Arizona license as a registered nurse, current certification in appropriate nurse practitioner specialty, prescribing privileges, Drug Enforcement Administration Certification (DEA),

Additional Requirements:
Basic Life Support Certification.
Must possess a current fingerprint clearance card issued by the Arizona Department of Public Safety.

Knowledge of:

  • Psychiatric medicine, including assessment, prescribing medications, conducting appropriate tests, determining, and implementing treatment plan and evaluation of therapy.
  • Health center operations and patient care management including formulary, utilization management and fiscal oversight.
  • Business English, spelling, grammar, punctuation, and composition.
  • Clerical, office practices and procedures.
  • Research techniques and report writing.
  • Current MS office software.
  • Filing and recordkeeping.
Skill in:
  • Utilizing leadership skills to create a workplace environment that fosters open communication, teamwork, and professionalism
  • Communication and supervisory skills.
  • Communicating effectively verbally and in writing.
  • Communicating orally in a face-to-face setting and by telephone with patients, public and employees.

Ability to:
  • Travel to outlying areas within Yavapai County.
  • Document in accordance with regulations for licensure in Arizona, State and Federal Program Guidance Manuals and the requirements of payors.
  • Utilize the electronic medical record for patient management.
  • Develop and maintain effective working relationships with patients, associates, employees of other departments, representatives of other organizations, County officials, and the public.
  • Keep management informed of key operating issues affecting the department.
  • Collaborate with administration and other CHCY staff in establishing goals, policy and procedures and work processes.
  • Participate in work related initiatives and meetings.
  • Remain current in knowledge required to perform assigned duties.
  • Handle confidential matters and maintain discretion always.
  • Work professionally and effectively with co-workers, clients, victims of crime, witnesses, and the general public.
  • Act with courtesy, tact, and diplomacy
Work Environment:
The work environment is associated with an office. The work area is adequately lighted, heated or cooled and ventilated. The work involves potential exposure to infections, contagious diseases and blood borne pathogens. Clinical staff are required to use appropriate protective equipment. Subject to varying and unpredictable situations.

Physical Requirements:
Ability to sit or stand for extended periods of time. Sufficient vision to read printed materials and a computer screen; hearing and speech ability to communicate effective in-person and telephonically. Capable of frequent repetitive movement of arms, fingers, and wrists. Ability to lift up to 30 pounds.
