Job Description

The Tulare County Health & Human Services Agency (HHSA) is currently recruiting for a Supervising Nurse I with leadership skills to join the Integrated Services Division. This position is responsible for supervising a team of up to eight paraprofessionals that include case managers and community health workers. The team will work across branches as well as with external stakeholders such as managed care plans, primary care, and community organizations to lead a team that provides member-centered, coordinated care utilizing trauma-informed care best practices. The current vacancy is in the Integrated Services Unit; however, this recruitment will establish an employment list to fill immediate and future vacancies in the Administration, Public Health, and Human Services Branches, which includes the Integrated Services Division, Tulare County Health Centers, the Public Health Nursing and Prevention Programs, and the Child Welfare Services (CWS) Division.

Incumbents working in the Integrated Services Unit will be well versed in providing leadership, supervision, and training to a team of paraprofessional staff in roles such as case management, health education, and program navigation. Team members will have experience in various subjects that impact enrollees such as homelessness, addiction, prior incarceration, severe trauma, and multiple chronic health conditions. The successful candidate will be leading multi-disciplinary team meetings that may include any of the following: the patient, unit staff, representatives from primary care, behavioral health, social services, probation/parole, housing services, foster care, and other similar case management programs in government and/or community organizations. The objective of these meetings is to establish a care plan and coordinate services across multiple programs and service providers to meet care plan goals for the duration of the patient’s enrollment. The successful candidate must have the ability to maintain effective working relationships with patients, the public, staff, and other departments and agencies.

The Public Health Branch protects and promotes the health status of Tulare County residents through the development and implementation of public health and primary care programs that use best-practice interventions. Incumbents working in the Public Health Nursing and Prevention Programs will work with public health programs with a primary emphasis on California Children’s Services programs, Tuberculosis, Immunizations, General Public Health Nursing, Communicable Diseases, Maternal, Child & Adolescent Health, and Sexually Transmitted Diseases (STD)/ Human Immuno-Deficiency Virus (HIV) programs. The incumbent’s primary role will be to act as a liaison between the local health jurisdiction and the State. The selected candidate will also be in a leadership role within the Health Branch, working closely with the Health Officer as well as the Director of Public Health Nursing. The Supervising Nurse at the County Health Centers will work closely with the clinic manager to ensure the coordination of services among the medical, nursing, and support staff within the clinic. The ideal candidate will have the ability to communicate effectively and clearly with patients and with various providers on behalf of patients. In addition, the successful candidate will act as a liaison between HHSA, patients, and their families. The candidate will closely work with clinic administration staff in promoting quality patient care and practices to the community and will also collaborate with other HHSA programs in developing policies and protocols that protect and strengthen the well-being of the community.

The Child Welfare Services (CWS) Supervising Nurse will provide supervision, guidance, and overall direction for the CWS nursing program and will assist and ensure compliance with the preventive health and dental exams for all children in out-of-home care. The incumbent will supervise Public Health Nurses and CWS Registered Nurses who consult with social workers in their investigation of allegations of child abuse or neglect. The candidate will represent CWS in state and regional meetings and serve as a liaison and build a cooperative partnership with external programs.

Typical Duties

Minimum Qualifications

Candidates must meet the qualifications listed below by the application deadline .

  • Graduation from an accredited college or university with a Bachelor of Science degree in nursing
  • Three years in community health or primary care nursing, including one year in a supervisory capacity
  • A valid California Registered Nurse License

OR
  • Graduation from an accredited college or university with an AA degree in nursing
  • Five years of experience in community health or primary care nursing, including two years in a supervisory capacity
  • A valid California Registered Nurse License

DESIRABLE QUALIFICATIONS
  • Experience working in whole-person care, health homes, or any CalAIM initiatives.
  • Bilingual: English/Spanish

Conditions of Employment
  • Candidates selected will be required to pass a pre-employment drug and alcohol screening. Additionally, a background investigation may also be conducted, which may include a re-investigation every 10 years for some positions. Retirement:
The retirement plan is a defined benefit plan administered pursuant to the 1937 Act County Employees Retirement Act and integrated with Social Security. In addition to ordinary retirement benefits, the plan provides disability and death benefits. Retirement contributions are made by both the County and the employee. The County has reciprocity with the State of California, contracting PERS agencies, and all County 1937 Act Retirement Systems.

Paid Holiday Leave: 12 set days and 1 personal holiday.

Vacation Accrual:

2 weeks per year (0-3 years of service)
3 weeks per year (3-7 years of service)
4 weeks per year (7-11 years of service)
5 weeks per year (11+ years of service)
Limit of 300 hours.

FLSA exempt employees accrue an additional 5 days of vacation per year.

Sick Leave Accrual: 12 days per year with unlimited accumulation, 50 hours of which may be used toward family sick leave.

Group Term Life Insurance and AD&D: $10,000; Provided by Standard Insurance Company.

Disability Insurance:
Employees are covered by State Disability Insurance. The premium is paid by the employee.
