CalOptima Health is seeking a highly motivated an experienced Physical Therapist (PT) to join our team. The Physical Therapist for the Program for All-Inclusive Care for the Elderly (PACE) evaluates and assesses participants to determine mobility level and other physical conditions and functional abilities. The incumbent will provide and recommend treatment protocols such as therapeutic exercise, functional mobility training and safety education and complete assessments of proper durable medical equipment (DME). The incumbent will plan and develop individual and group exercise programs for maintenance therapy for participants. The incumbent will be responsible for providing a comprehensive physical therapy program for participants of PACE, including development and oversight of functional maintenance programs.

Duties & Responsibilities:
  • 80% - Program Support
    • Participates in a mission-driven culture of high-quality performance, with a member focus on customer service, consistency, dignity and accountability. Assessment:
      • Collects participant data and completes required forms with appropriate responses according to department and industry standards.
      • Identifies participant's overt problems/needs and sets priorities.
      • Identifies problems requiring further referral and/or follow-up.
      • Observes and records the latest diagnostic results.
    • Planning Care:
      • Develops a plan of care based on physical therapy processes which incorporates the plans of other disciplines and continuing care needs.
      • Includes the participant/family in developing or revising the plan of care and makes referrals to interdisciplinary support services.
      • Establishes realistic and measurable goals and sets up treatment plans to administer rehabilitation treatment.
      • Completes initial and periodic assessments (minimally every 6 months) prior to the scheduled team meetings.
      • Communicates participant changes to team members.
    • Implementing/Managing Care:
      • Ensures the therapy being provided conforms to accepted industry practice standards and organizational/department standards.
      • Provides evidence-based treatments/procedures and other care as prescribed and according to participant care standards.
      • Demonstrates thorough understanding of age-related characteristics, symptoms and needs of participants served.
      • Explains physical therapy treatments and procedures in an appropriate way.
      • Provides participant and family education in rehabilitation needs and interventions.
      • Works with other therapists and primary care staff to provide participant care and to implement physician orders, as necessary and as appropriate, to physical therapy standards.
      • Documents all physical therapy treatment accurately and within departmental time requirements into participant's charts.
      • Plans and develops maintenance exercise and therapy programs for participants.
    • Participant/Family Education:
      • Assists and works with the Interdisciplinary Team (IDT) on delivery of care to provide training and education to family members, caregivers and PACE staff for proper carryover of treatment interventions.
      • Makes home visits for home-based interventions and observation.
    • Evaluation:
      • Records participant care delivered as planned and any variations, with appropriate rationale.
      • Makes and records observations, as well as implements rehabilitation measures, related to impending or associated problems.
    • Leadership:
      • Provides supervision to program aides and other staff who are implementing a physical therapy treatment and/or maintenance program.
      • Integrates cost effective measures into rehabilitation practice.
      • Recognizes unit problems and takes responsibility for documentation.
      • Complies with policies addressing safe working conditions; monitors unsafe working conditions and recognizes inappropriate and/or ineffective participant care management.
      • Resolves issues/problems; completes written reports; fulfills call for department as assigned and adjusts staffing for volume and acuity as necessary.
    • Interdisciplinary Team:
      • Participates as an active and cooperative member of the IDT.

  • 15% - Administrative Support
    • Assists the team in carrying out department responsibilities and collaborates with others to support short- and long-term goals/priorities for the department.
    • Attends staff meetings and takes part in participant care planning.

  • 5% - Completes other projects and duties as assigned.

Minimum Qualifications:
  • Bachelor's degree in physical therapy required.
  • 1 year of experience working with the frail or elderly population as a physical therapist required.
  • An equivalent combination of education and experience sufficient to successfully perform the essential duties of the position such as those listed above may be also qualifying.
  • Have access to means of transportation for work away from the primary office approximately 10% of the time to provide continuum of care.

Preferred Qualifications:
  • Master's degree in a related field.

Required Licensure / Certifications:
  • Valid and current California Physical Therapy License, in good standing, required.
  • CPR and First-Aid Certification required or must be obtained within six (6) months of date of hire.

Knowledge & Abilities:

  • Develop rapport and establish and maintain effective working relationships with CalOptima Health's leadership and staff and external contacts at all levels and with diverse backgrounds.
  • Work independently and exercise sound judgment.
  • Communicate clearly and concisely, both orally and in writing.
  • Work a flexible schedule; available to participate in evening and weekend events.
  • Organize, be analytical, problem-solve and possess project management skills.
  • Work in a fast-paced environment and in an efficient manner.
  • Manage multiple projects and identify opportunities for internal and external collaboration.
  • Motivate and lead multi-program teams and external committees/coalitions.
  • Utilize computer and appropriate software (e.g., Microsoft Office: Word, Outlook, Excel, PowerPoint) and job specific applications/systems to produce correspondence, charts, spreadsheets, and/or other information applicable to the position assignment.

Physical Requirements (With or Without Accommodations):
  • Ability to visually read information from computer screens, forms and other printed materials and information.
  • Ability to speak (enunciate) clearly in conversation and general communication.
  • Hearing ability for verbal communication/conversation/responses via telephone, telephone systems, and face-to-face interactions.
  • Manual dexterity for typing, writing, standing and reaching, flexibility, body movement for bending, crouching, walking, kneeling and prolonged sitting.
  • Lifting and moving objects, patients and/or equipment more than 50 pounds

  • Work is typically indoors and sedentary and is subject to schedule changes and/or variable work hours, with travel as needed.

If located at PACE:
  • Work is typically indoors in a clinical setting serving the frail and elderly.
  • There may be harmful or hazardous environmental conditions present for this job.
  • The noise level in this work environment is usually moderate to loud.

If located in the Community:
  • Work is typically indoors and sedentary and is subject to schedule changes and/or variable work hours, with travel as needed.
  • Employee will occasionally work outdoors in varied temperatures.
  • There may be harmful or hazardous environmental conditions present for this job.
  • The noise level in this work environment is usually moderate to loud.
