Job Location: This position will be located at a base location that is flexible within the State of Missouri. This position is telecommuting-eligible if candidate meets all telecommuting eligibilities and qualifications.

The salary indicated represents a base pay rate. If the individual selected or the position is eligible for a pay differential (e.g., shift, security, or years of service), it will be added to the total compensation in your paycheck. A pay differential does not raise your base pay.
What you’ll do: - Lead phone interviews to gather pertinent information to initiate service referrals and change requests.
- Determine preliminary MO HealthNet and HCBS programmatic eligibility.
- Review, monitor, and update information in various electronic systems and databases.
- Coordinate with a blended team of professionals to ensure quality care.
- Provide objective and accurate documentation.
- Classify and generate actions for multiple HCBS teams.
- Serve with exceptional professionalism in a contact center environment.
Minimum Qualifications - High School diploma or equivalent and 0-2 years of relevant experience. (Substitutions may be allowed.)
- Strong interviewing and communication skills.
- Keen to detail and documentation.
- Technical proficiency.
- Exhibit disability & cross-generational awareness.
- Prior experience with customer service, call centers, and working with the elderly or persons with disabilities.
