    Ventura County’s Ambulatory Care clinics serve underserved and vulnerable communities through high-quality, compassionate care. In partnership with Public Health's Comprehensive Perinatal Services Program (CPSP), we provide vital support to Medi-Cal-eligible pregnant and postpartum women-including health education, nutrition counseling, and psychosocial services-to promote healthier pregnancies and birth outcomes.

    We’re seeking a bilingual (English/Spanish) Lead Comprehensive Perinatal Services Worker to assess patient needs, coordinate care, educate and counsel pregnant patients, and their families and support CPSP implementation across our clinics. This role also includes training staff and contributing to service improvements that help deliver equitable care during a critical life stage.

    The ideal candidate has experience working with perinatal populations, is skilled in patient education, case management, and counseling, and has contributed to the development of workflows or training materials in a healthcare setting. A strong commitment to maternal and infant health and health equity is essential.

    WHAT WE OFFER:
    The County of Ventura offers an attractive compensation and benefits package. Aside from our base salary range, an employee within this position may also be eligible for the following:

    • Educational Incentive - An educational incentive of 2.5% for completion of an associate's degree, 3.5% for completion of a bachelor's degree, or 5% for completion of a graduate's degree.
    • Bilingual Incentive - Incumbents may be eligible for bilingual incentive depending upon operational need and certification of skill. Proficiency levels by exam are $0.69/hour (Level I), $1.00/hour (Level II), or $1.32/hour (Level III).
    • Merit Increases - New employees are eligible for an initial 5% merit increase upon completion of at least 1,040 hours (approximately 6 months) assuming work meets satisfactory standards. Subsequent merit increases will be upon completion of each additional 2,080 hours (approximately one year) from the initial merit increase.
    • Deferred Compensation - Eligible to participate in the County's 401(K) Shared Savings Plan and/or the Section 457 Plan. If you participate in the County sponsored 401(K) plan, the County will match a part of your contribution up to 3% of your salary, on a per pay period basis.
    • Health Plans - you are afforded a flexible credit allowance for purchasing medical, dental, and/or vision insurance.
    • Flexible Spending Accounts - Pre-tax benefit towards eligible medical, dental, and vision care expenses.
    • Pension Plan - Participate in the County's defined benefit pension plan that provides lifetime payments to retirees. Members are "vested" after attaining five years of retirement service credit. If eligible, you may establish reciprocity with other public retirement systems such as CalPERS.
    • Holidays - 12 paid days per year which includes a scheduled floating holiday.

    PAYROLL TITLE: Community Services Coordinator

    DEPARTMENT/AGENCY: Health Care Agency - Ambulatory Care

    Community Services Coordinator is represented by the Services Employees International Union (SEIU) in an EEOC professional classification and is not eligible for overtime compensation.

    The eligible list established from this recruitment may be used to fill current and future Regular (including Temporary, Fixed-Term), Intermittent and Extra Help vacancies for this position only. There is currently one regular full-time vacancy within Ambulatory Care

    Examples Of Duties

    Duties may include but are not limited to the following:
    • Plans, organizes and coordinates program and staff activities including case management, records management, and community outreach activities;
    • Trains, orients and prepares training for other CPSWs in the ambulatory care clinics;
    • Serves as a resource by providing consultation and guidance to other CPSWs;
    • Distributes and assigns work;
    • Establishes work sequence and procedures for others;
    • Designs/drafts changes to improve work flow;
    • Assists supervisor in hiring/disciplining/evaluating others and determining work unit priorities by making suggestions and recommendations;
    • Contributes feedback or documentation to support performance evaluations;
    • Travels to other work sites as needed to provide support, coordinate services, attend meetings, or monitor program implementation;
    • Conducts needs assessments, initial screening and social histories of pregnant clients referred to the program;
    • Develops, administers, supervises and evaluates health education, counseling or treatment programs for prenatal population;
    • Developing and updating care plans based on assessments; prioritizing needs, and setting goals;
    • Provides case management and care coordination to pregnant patients and their families;
    • Connects clients with other relevant services or community resources for support and assistance;
    • Facilitates communication and collaboration between different healthcare providers and services;
    • Serves as liaison with representatives from state, County, and local agencies, school districts, courts and the community to identify and resolve problems/needs;
    • Interprets and implements federal, state and County laws, regulations, and policies to insure adherence to state and County program and fiscal guidelines as they relate to the CPSP program;
    • Prepares a variety of reports and correspondence; and
    • Performs other duties as required.


    Typical Qualifications

    These are entrance requirements to the exam process and ensure neither continuance in the process nor placement on an eligible list.

    EDUCATION, TRAINING, and EXPERIENCE:
    Considerable experience in social work, community health, mental health or education which has led to the acquisition of the required knowledge and abilities.

    The required knowledge and abilities also can be obtained by four (4) years of experience in social work, community health, mental health or education, including responsibility for program development and implementation.

    NECESSARY SPECIAL REQUIREMENTS:
    • One (1) year of the qualifying experience must be working with a perinatal population within a healthcare setting
    • Bilingual in English/Spanish
    • Possession of a Comprehensive Perinatal Services Program (CPSP) Certification issued by California Department of Health Care Services (DHCS)
    • Experience in process improvement and workflow development, including evaluating current practices and implementing enhancements for efficiency and quality
    • Experience conducting classroom training and educational workshops
    • Intermediate skills using Microsoft Word, Excel and PowerPoint


    DESIRED:
    • Experience providing services to the community
    • Public speaking experience

Using a T&E, applicants may be scored or ranked according to criteria that most closely meet the business needs of the department.     Applicants successfully completing the exam process may be placed on an eligible list for a period of one (1) year.
