OverviewThis position will provide direct care/health services in the TB Clinic for LTBI patients to include but not limited to:
* Care management of patients suspected of LTBI or diagnosed with latent tuberculosis to include obtaining and evaluating medical history, performing patient assessments, collecting appropriate lab specimens, reviewing lab results, administering medication and consulting/collaborating with the physician and other team members.
* Provide treatment, education, health counseling and appropriate referrals to LTBI clinic clients.
* Identify actual and potential healthcare needs of patients, as well as, healthcare access or treatment barriers.
This position will manage our clinic’s Class D pharmacy to include:
* Ordering medication for all TB Clinic clients from DSHS .
* Maintaining appropriate levels of medications in our clinic’s Class D pharmacy.
* Preparing weekly medication packets for all our TB Clinic clients.
* Keep up-to-date necessary records to maintain our Class D Pharmacy compliance with DSHS .
This position will require skills in establishing effective and constructive relationships with patients, families, physicians, coworkers and the general public.
This position will require skills in effective oral and written communication, educating and counseling patients.
This position will perform data entry duties as needed to include monthly performance measure reports.

Duties, Functions and Responsibilities
Essential duties and functions, pursuant to the Americans with Disabilities Act, may include the following. Other related duties may be assigned. Use a systematic approach to provide individualized, goal-directed nursing care using data collection and focused nursing assessments. Provide general nursing care for ill, injured or convalescent and/or handicapped patients in clinics or private homes. Assist in determining the health status and needs of patients based on interpretation of health related data and preventative health practices. Assist in the formulation of goals/outcomes and plan of care in collaboration with the patient, their families and healthcare team members. Implement teaching plan for patients with common health problems and well-defined learning needs. Assist in the evaluation of the patient’s responses and outcomes to therapeutic interventions. Provide direct nursing care to patients seen in the CHC setting. Participate in the identification of patient needs for referral to resources that facilitate the continuity of care. Distribute oral medications and treatments according to accepted standards of care. Accurately and completely report and document status, care rendered, response to care, provider orders, contacts with patient and/or other healthcare providers. Assist with the orientation of new staff. Participate in implementing changes that lead to improvements in the work setting. Report unsafe care environment and equipment.
