    DESCRIPTION

    The Community Program Specialist III (CPS) for The Nleta Project plays a pivotal role in facilitating the implementation, coordination, and administration of the holistic home visitation program designed to support Black Infant Health Program Graduates. This position focuses on cultivating and managing stakeholder partnerships, assisting with client intake processes, developing data collection tools, and ensuring timely submission of programmatic reports to support program effectiveness and compliance.

    EXAMPLES OF DUTIES

    • Implement and maintain a culturally relevant program that respond to the values of the Black/African American community.
    • Identify, cultivate, and maintain partnerships with community organizations, healthcare providers, government agencies, and other stakeholders to enhance program visibility, resources, and support.
    • Collaborate with stakeholders to coordinate referrals, access additional services, and advocate for the needs of program participants and their families.
    • Assist in the client intake process by gathering demographic information, assessing needs, and coordinating initial assessments with program staff.
    • Support families in completing necessary paperwork, obtaining consent for services, and accessing relevant community resources as needed.
    • Develop data collection tools and protocols to capture program outcomes, participant demographics, service utilization, and other relevant data elements.
    • Ensure accurate and timely entry of data into program databases or electronic health record systems.
    • Monitor data quality, conduct regular audits, and implement corrective actions to address discrepancies and improve data integrity.
    • Prepare and compile programmatic reports, including progress updates, performance metrics, and outcome measures, in accordance with grant requirements and reporting deadlines.
    • Collaborate with program leadership and evaluation team members to analyze data, identify trends, and communicate program impact to stakeholders.
    • Assist in the development of grant proposals, funding applications, and presentations to secure additional resources and support for program expansion and sustainability.
    • Provide training and technical assistance to program staff and partners on data collection procedures, reporting requirements, and best practices for program administration.
    • Offer support and guidance to ensure staff competency in utilizing data systems, maintaining confidentiality, and adhering to relevant policies and procedures.
    • Implement quality assurance measures to monitor program fidelity, adherence to protocols, and compliance with regulatory standards.
    • Participate in program evaluations, performance reviews, and quality improvement initiatives to identify areas for enhancement and ensure alignment with program goals and objectives.
    • Assist in the development and dissemination of promotional materials, outreach strategies, and communication campaigns to raise awareness about the Nleta Project and its services.
    • Represent the program at community events, meetings, and forums to engage stakeholders, solicit feedback, and foster collaborative partnerships.


    REQUIREMENTS TO FILE

    EDUCATION:
    • Bachelor's Degree in Business Administration, Public Administration or a closely related field (Proof of education required).
      • Professional experience beyond the minimum qualifications may be substituted for the required education on a year-for-year basis.

    EXPERIENCE:
    • A minimum of two (2) years of full-time professional experience in the development and implementation of grant-funded programs, projects and services.
      • A Master’s Degree in Business, Public Administration, or a closely related field from an accredited college or university may be substituted for up to one (1) year of the required professional experience.

    ADDITIONAL REQUIREMENTS:
    • A valid motor vehicle operator’s license may be required for some positions.
      • Required to obtain a California motor vehicle operator license within 6 months if out of state. DMV K4 driver license information report printout required during onboarding for California residents.
    • Some positions may require working overtime, weekends, and evenings.
    • Some positions may require bilingual skills (e.g.: Spanish, Tagalog or Khmer).

    DESIRABLE QUALIFICATIONS

    • Master's degree or master's degree level coursework completed in the health or human services field.
    • Lived experience in communities served by the Black Infant Health Program or experience navigating similar social, health, or economic challenges is highly valued.
    • Knowledgeable about community organizations and community resources serving the Black/African American community.
    • Demonstrates an understanding of the complex, interrelated issues and concerns contributing to health disparities affecting African Americans.
    Ability To:
    • Learn, interpret, retain, and communicate technical and complex information, terminology, policies and procedures related to grant programs and compliance.
    • Research and analyze data using a systematic and logical approach.
    • Communicate clearly and concisely, both orally and in writing.
    • Understand and follow oral and written instructions and procedures.
    • Maintain accurate records and files.
    • Work independently and/or as part of a team.
    • Work effectively with a wide variety of people by consistently exercising tact, good judgment and a pro-active, problem-solving focused communication style.
    • Operate office equipment including computers, printers and copiers as well as the word processing, spreadsheet, e-mail, database and program specific applications.
    • Type and enter data accurately.
