    DESCRIPTION

    T he City of Long Beach Department of Health and Human Services, Community Health Bureau, is recruiting for a full-time unclassified Health Educator II to work in the Tuberculosis Clinic. Reporting to a Public Health Nurse III, the Health Educator II will perform a variety of duties related to Tuberculosis prevention and control.

    EXAMPLES OF DUTIES

    • Conduct field investigation to locate and contact those who have or are likely to have been expose to TB.
    • Role requires driving to various locations as part of the job duties. A valid driver’s license and reliable transportation are necessary
    • Refers patients and conducts medical quality assurance activities to ensure examination and proper treatment for those with TB infections;
    • Provides linkages to care including medical and mental health services.
    • Maintain case files, and program documentation.
    • Provides education on Tuberculosis to clients.
    • Provides and assists clients with community referrals, resource information and access, and linkages to support to TB testing, treatment, and other support services;
    • Trains and disseminates information to clients.
    • Develops and disseminates health content to the public.
    • Review case files, conduct peer reviews and participate in case review meeting and huddles, and studies for quality assurance/control.
    • Conduct TB investigation during TB outbreak.
    • Utilizes RedCap, CalREDIE and NextGen data registry to update patient files, report services, complete progress notes, and monitor program activities.
    • Conducts community outreach to recruit for testing.
    • Participates in agency and community meetings, forums, and trainings; and
    • Provides Directly-Observed Therapy (DOT)
    • Performs other related duties as required.


    REQUIREMENTS TO FILE

    EDUCATION
    • High School diploma or equivalent and

    EXPERIENCE
    • Two years experience in a health education setting (Education may be substituted for experience on a year-for-year basis)

    ADDITIONAL REQUIREMENT
    • A valid California motor vehicle operator’s license is required.

    DESIRABLE QUALIFICATIONS:
    • Two (2) years of professional experience working for a Tuberculosis program.
    • Bilingual skills (Khmer, Spanish, and/or Tagalog).

    SUCCESSFUL CANDIDATES WILL DEMONSTRATE:
    • Ability to work independently, exercise sound judgment, be flexible, and manage multiple assignments within a fast-paced working environment.
    • Excellent written, verbal, and interpersonal communication skills.
    • Ability to maintain confidentiality and knowledge of California HIPAA regulations. Experience working with diverse groups and populations.
    • Knowledge of community services in the Long Beach and Los Angeles areas.
    • Proficiency in the use of computers including email, internet, and office productivity software (e.g. Microsoft Word and Excel and electronic health record systems).
