    POSITION SUMMARY

    The Community Health Coordinator for the Mobile Integrated Health Program plays a key role in enhancing community health by collaborating with community members, medical providers, and local resources. The primary focus is to reduce opioid overdose incidents by connecting patients with appropriate services and identifying effective health solutions. Responsibilities include documenting patient assessments, tracking contact, submitting weekly reports, and acting as a liaison between community members and medical services. This role involves identifying barriers to medical care and assisting individuals in navigating the necessary channels to access services, including social and psychological support. The coordinator will also promote community health and wellbeing through education and outreach, address identified risk areas and facilitate the coordination of essential services.
    **This is a Grant-Funded Position**

    This job classification is in Management Category IV for benefits purposes

    ESSENTIAL JOB FUNCTIONS

    Essential functions are the basic job duties that an employee must be able to perform, with or without reasonable accommodation.     • Contribute to the development and refinement of programs funded by the CORE grant, ensuring they are responsive to community needs.
    • Raise awareness about opioid misuse, treatment options, and resources available for individuals and families affected by addiction.
    • Evaluate client’s substance abuse challenges and social needs through interviews and other assessment tools developing a plan to address client specific needs.
    • Support clients in accessing essential resources and services, including medical treatment, counseling and social support.
    • Help clients navigate the healthcare system and advocate for their needs to ensure they receive appropriate care and support.
    • Work with healthcare providers, social workers, peers and other professionals to ensure clients receive comprehensive care.
    • Keep accurate records of client interactions, treatment plans, and outcomes, ensuring compliance with grant requirements and confidentiality regulations.
    • Assist in preparing reports for grant compliance, detailing program activities, successes, challenges, and areas for improvement
    • Provide culturally competent care and ensure that services are accessible to individuals from diverse backgrounds and communities.
    • Identifies needs of the community members and coordinates health and community services.
    • Motivates community members/clients and their families to be active and engage participants in their health and overall wellbeing to reduce 911 misuse, decrease hospital readmission.
    • Collaborate with other community organizations, health departments, and law enforcement to build a network of support for individuals affected by opioid addiction.
    • Conduct Community Events and informational sessions to the public about substance abuse, harm reduction strategies, and available resources.
    • Create and Implement care plans in collaboration with the Mobile Integrated Health Team, addressing both immediate and long0term needs.
    • Drives and operates city vehicle and other response vehicles.
    • Operates a two-way radio.
    • May be required to be on-call, and available to respond to emergency scene.
    • Work requires a flexible schedule including some nights and weekends.
    • Performs related work as required.


    JOB REQUIREMENTS & WORK ENVIRONMENT

    Bachelor’s degree in Social Work, Public Health, Health Services, or a related field.Three (3) to five (5) years of related work experience providing social services and/or health education and outreach to the community. Additional qualifying experience or completion of coursework at an accredited college or university in a job-related field, may substitute on a year-for-year basis for the required experience or education.Possess or be able to obtain an American Heart Association Heartsaver First Aid CPR AED or equivalent, within 30 days of hire. Possess or be able to obtain a valid State of Florid Driver's License and have an acceptable driving record, within 30 days of hire.All required certifications and licenses, whether possessed at time of appointment or obtained after appointment, must be kept current and valid while employed in a position in this class.
    Preferred
    • An American Heart Association BLS CPR & AED.
    • Certified Community Health Worker (CCHW).


    As public servants, employees may be required to work immediately before, during, and/or after an emergency. All employees must be available and able to work assigned shifts as determined by their Department Heads.

    PHYSICAL STANDARDS REQUIRED TO PERFORM ESSENTIAL JOB FUNCTIONS:

    The position involves medium physical demands, such as exerting up to 50 lbs. of force occasionally, and/or up to 20 lbs. of force frequently, and/or up to 10 lbs. of force constantly to move objects.
