    Summary of Position

    The Health Department’s Public Health (PH) Bureau is seeking to fill one (1) full-time Supervising Public Health Nurse (SPHN) position in the Enhanced Care Management (ECM) program.

    The Supervising Public Health Nurse (SPHN) coordinates and supervises the operation of health-related services and programs involving comprehensive case management, education, prevention, and collaboration with and coordination of community resources. These services are delivered by a team of professional, para-professional and support staff that collaborate to ensure comprehensive health services are provided throughout Monterey County. The supervisor uses principles of reflective practice and staff development to provide primary support and appropriate oversight to the nurse home visitors to build and model a strengths-based client-centered culture.

    The ECM Program serves clients who are experiencing homelessness, have chronic medical conditions, are high utilizers of the emergency department and/or hospitalizations, and/or have severe mental health diagnosis. As part of the CalAIM initiative, the Bureau's ECM program is expanding to serve a larger proportion of individuals in Monterey County eligible for services. The SPHN will lead a multidisciplinary team of Community Service Aide IIs, Behavioral Health Aides, Licensed Vocational Nurses, and Public Health Nurse IIs. In this position, the incumbent will hire and train new team members, assign and review the workload of the team, and coordinate with partners. This will include the evaluation of program effectiveness, and provide technical expertise and recommend and implement policies and procedures to ensure ECM’s daily operations comply with both long and short-term program goals and objectives and agency policies and Procedures. Additionally, the incumbent will also assist the PH Program Manager track expenditures and draft reports.

    As of September 8, 2023, the Health Department may offer up to $10,000 signing bonus to candidates hired into regular full-time positions for this classification from this recruitment hired after September 8, 2023. Exam Plan 24/52E80/08PH.

    The Eligible List established by this recruitment process may be used to fill current and future vacancies on a regular full-time, part-time, or temporary basis County-wide.

    Examples of Duties

    • Plans, assigns, and reviews the work of staff. Assists in hiring staff and recommends disciplinary action when appropriate; approves staff leave requests. Evaluates staff training needs and develops strategies for addressing training needs; promotes team building.
    • Coordinates program operations with other county departments as well as other agencies.
    • Coordinates, supervises, and directly participates in carrying out a variety of complex and varied health services programs.
    • Assesses and anticipates program needs and recommends and implements policies and procedures designed to assure that daily operations comply with both long and short-term program goals and objects and agency policies and procedures.
    • Provides technical expertise concerning the medical, nursing, technical, professional and or administrative elements of programs; evaluates and makes decisions on difficult programs or admirative problems.
    • Prepares annual program planning documents; analyzes and provides cost and revenue input for budgets, grants, etc. Administers grant funds and budgets; evaluates financial aspects of programs relative to approved budgets and service plans. Participates in developing division objectives and priorities.
    • Establishes methods for evaluating program effectiveness.
    • Serves on relevant committees and task forces, as assigned; serves as liaison to state and other governmental agencies.
    • Develops and submits a variety of management reports and program evaluation.

    THE SUCCESSFUL CANDIDATE

    Will have a proven track record demonstrating the following knowledge, skills, and abilities:
    Thorough knowledge of:
    • Principles, methods and procedures of public health nursing.
    Working knowledge of:
    • The principles and practices used in public health program coordination and implementation.
    • The principles and practices used in basic budget and grant administration.
    • The laws, codes and standards applicable to specific public health programs.
    • Community aspects, needs and resources relative to specific public health programs.
    • Principles and practices of staff supervision and development.
    Some knowledge of:
    • In addition, depending on the particular public health programs assigned, each position in this class will require knowledge of the principles, techniques, methods and terminology used in specific medical specialties (e.g., maternal/child health, immunization, perinatal, communicable disease, STD’s, etc.).
    Skills and Ability to:
    • Analyze, evaluate, and draw logical conclusions from data; evaluate problems, policies, procedures and formulate an effective course of action.
    • Think creatively; develop new methods, procedures, or approaches to achieve program goals; exercise initiative ingenuity and sound judgment in identifying and solving difficult administrative and technical problems.
    • Plan, coordinate and implement overall work plans to achieve a designated objective; coordinate multi-faceted projects and studies, including coordination and planning for resources, manpower and timing.
    • Plan, directs and evaluate the work of others.
    • Speak effectively before groups for the purpose of soliciting and providing information and/or persuading others to pursue a specific course of action.
    • Write effectively to produce a variety of reports, correspondence and advertisement materials.
    • Establish and maintain cooperative work relationships with those contacted through the course of work; interface with community and agency representatives regarding issues of a sensitive or political nature.
    • Work independently with little or no direct supervision.
    • Understand, interpret and apply rules, laws, policies and regulations relative to specific public health programs.
    • Follow oral and written instructions.


    Examples of Experience/Education/Training

    Any combination of training, education, and/or experience which provides for the knowledge, skills, and abilities required to perform the duties listed above is qualifying. An example of a way these requirements might be acquired is:

    Education: Completion of coursework leading to a Bachelor’s Degree in Nursing with a certificate in Public Health Nursing which included courses in the principles of supervision and administration and public health.

    AND

    Experience: Two years of progressively responsible professional public health program experience which include lead-worker level responsibilities, and which provide a broad knowledge of public health programs.

    CONDITIONS OF EMPLOYMENT

    The required conditions of employment include, but are not limited to the following:
    • Pursuant to Title 17 of the California Administrative Code, possession of a license in active status, to practice as a registered nurse in California and possession of a Public Health Nursing Certificate.
    • Possess and maintain a valid California Class C driver’s license or the ability to provide suitable transportation that is approved by the appointing authority.
    • Be available to work a flexible schedule, including evenings, weekends, holidays, and during times of disaster and/or emergency.
    • Successfully complete a background check to include fingerprinting.
