The social worker will be responsible for maintaining a caseload of approximately 8 cottage-based clients within Ward H of the Social Learning Program (SLP). Responsibilities include provision of individual and group therapy/psychoeducation, community reintegration activities, transfer and discharge coordination, division specific documentation, case management, liaising with legal guardians and family where applicable, and other duties as assigned. Within SLP’s case management model, the social worker will be responsible for additional tasks, including but not limited to, ITRP review and Integrated Risks Assessments.

The social worker will also be required to participate in routine interdisciplinary meetings, including client review, treatment planning, and forensic release planning.

The Social Learning Program, as it implies, uses a social learning model across Ward H, Cottage D, Cottage 6 and 8. Social Learning was developed for individuals with profound deficits associated with severe and persistent mental illness or intellectual deficit disorders. SLP is a token-based economy that helps to develop skills that are necessary to live in the community.

    Responsibilities:

    • Conduct comprehensive Social Service Assessments (Initial and/or Annual)

    • Complete admission and discharge paperwork

    • Provide individual and/or group treatment and complete corresponding documentation

    • Create Individual Treatment/Rehabilitation Plans (ITRP) and Treatment Reviews and modify/update on a routine basis

    • Write monthly/weekly summary progress notes for clients

    • Offer case management to assigned clients

    • Conduct suicide risk and other risk assessments as needed

    • Collaborate with an interdisciplinary team on a routine basis

    • Participate in discharge and legal processes (e.g. guardianship, conditional release applications) as necessary

    • Disseminate pertinent information to vested departments within and outside the facility

    • Other duties as assigned (assignments will vary based on need and location)

    Eligibility:

    • Prior experience providing services for individuals with a severe and persistent mental health diagnosis and other impairing conditions (substance use disorders, personality disorders, intellectual disorders, etc.)

    • Working knowledge of the DSM, evidence-based practices and social work ethics are strongly encouraged.

    • Degree and/or License in Social Work (MSW, LMSW, or LCSW) preferred, but will consider candidates who meet the minimum qualifications.

    • Provisionally and fully credentialed candidates must be professionally licensed in Missouri.

    • Willing to consider individuals who are nearing completion of a MSW degree. This individual would likely qualify as a Clinical Caseworker until completion of MSW degree.

    • Lack of post-secondary education will not be used as the sole basis denying consideration to any applicant.
