DESCRIPTIONThe City of Long Beach, Department of Health and Human Services is recruiting a permanent, full-time, unclassified Public Health Professional I to serve as a Re-Entry Intake Specialist in the Collective Impact Bureau, Community Impact Division. Reporting to the Human Dignity Program Manager (Development Project Manager I), the Re-Entry Intake Specialist plays a vital role in empowering individuals impacted by the justice system as they transition from incarceration or other challenging circumstances back into their communities. This position focuses on providing tailored support by assessing individual needs, creating customized re-entry plans, and linking individuals and their families to critical resources and services with the goal of ensuring a smooth and successful reintegration. This position is housed at the Ron Arias Health Equity Center, located at 6335 Myrtle Avenue Long Beach, CA 90805.
EXAMPLES OF DUTIES- Oversees the initial comprehensive assessments of clients’ eligibility for services in line with the grant requirements.
- Develops individualized re-entry plans based on assessment findings and client goals.
- Assist in the development of a re-entry action plan to establish a one-stop location to effectively serve formerly incarcerated persons and their families.
- Develop re-entry program marketing plan and attends, hosts, and coordinates community events to increase awareness of existing services and support family reunification efforts for formerly incarcerated persons.
- Conducts case status convenings with funded service providers and grant advisory committee members.
- Serve as a liaison, along with the Human Dignity Manager, with county and state re-entry efforts to increase resources for community-based organizations to serve re-entry population in Long Beach.
- Assist with the development of grant proposals, contract management, and financial reporting.
- Assist with training and capacity building efforts among re-entry service providers and funded partners.
- Assist with the implementation of community restorative justice practices and trauma informed approaches to reduce family violence and strengthen community pathways for family reunification.
- Assist with the integration of re-entry services across city department efforts, including police community relationships.
- Act as a liaison between clients and service providers to ensure effective communication and coordination of services.
- Maintains accurate and confidential records of client interactions, progress, and service outcomes.
- Prepare and submit reports as required by funding agencies or organizational policies..
- Works closely with other professionals and organizations involved in the re-entry process, including probation officers, social workers, and community-based organizations.
- Organize, host, attend and participate in program related workshops, symposiums and trainings.
- Performs other duties as assigned.
REQUIREMENTS TO FILE EDUCATION - Bachelor's degree in Public Health, Community Health, Public Administration, Business, or closely related field. (Proof of education required)
- Experience offering specific and substantial preparation for the duties of the position may be substituted for the required education on a year-for-year basis.
EXPERIENCE - One-year of full-time equivalent professional experience in public health or human services related to a specialized program.
- A valid driver’s license, or the ability to arrange necessary and timely transportation for field travel.
- May require working overtime, weekends, and evenings.
- May require bilingual skills (e.g. Spanish, Tagalog, Khmer).
DESIRABLE QUALIFICATIONS - 2 or more years of experience, both personal and/or professional, in working with re-entry population and service provider networks.
- 2 or more years of experience with grant and/or project management.
- 2 or more years of experience with data analysis and setting up data metric dashboards.
- Experience in public speaking and excellent interpersonal, written, and verbal communication skills.
- Knowledge of racial equity and trauma informed approaches.
- Proficiency with office productivity software (i.e. Microsoft Office Suite, Excel, MS Word, etc.).
