Mental Health - Registered Nurse to join the
Cornerstone Community Support Team. The Community Support Team provides specialized services to clients who are engaged with primary Permanent Housing and Supportive Services (PHSS) programs. The
Mental Health - Registered Nurse works alongside primary case managers to improve quality of life and housing stability for clients who are or have been homeless and are living with severe mental illness, substance use disorders, and chronic health issues.
Essential Functions - Provides nursing care according to professional standards including assessment, service planning and implementation, injections and medication management
- Coordinates with clients and referring case managers to identify health needs and develop interventions to improve health outcomes
- Educates clients on heath care status and diagnoses to improve health functioning and encourage self-care activities
- Plans and implements nursing care and health activities based on client needs, preferences and available resources
- Arranges and facilitates health related educational groups
- Coordinates health events such as clinics and health fairs
- Follows Wake County Code of Conduct and adheres to strict confidentiality guidelines, including HIPAA
- Maintains required licensure for position by completing all continuing education requirements and following the standards of care/ethical code of the profession
- Provides services in the office, community and client homes

The Basics (Required Education and Experience) - Graduation from an accredited school of Nursing

- North Carolina Board of Nursing Licensed Registered Nurse
- CPR
- AED
- Equivalent education and experience are accepted

Beyond the Basics (Preferred Education and Experience) - Master's Degree
- 3 Year Mental health, psychiatry, working with special needs, chronically homeless populations
How Will We Know You're 'The One'? - Experience working with individuals who are experiencing homelessness and living with severe mental illness, substance use disorders, and complex health issues
- Ability to develop rapport and trust with clients
- Knowledge of Harm Reduction and Motivational Interviewing in the context of physical and mental health
- Ability to work as part of a multidisciplinary team
- Ability to adapt contents and teaching methods to client’s level of education, cognitive ability and learning style
- Ability to work independently and maintain professional boundaries
