    Job Location: This position will be located at a domicile in the St. Louis area, which may include St. Louis City, St. Louis County, or surrounding counties, with consideration given to surrounding areas based on the chosen applicant's location.

    The Bureau of Special Health Care Needs is seeking a qualified, compassionate, and dedicated candidate to fill a Registered Nurse/Service Coordinator position located in Eastern Missouri. This position will serve as service coordinator for the Healthy Children and Youth (HCY) and Medically Fragile Adult Waiver (MFAW) programs.

    • Conduct home visits and complete assessments to determine medical necessity in multiple Missouri counties.
    • Document interactions in a timely and accurate manner .
    • Prior authorize medically necessary in-home services.
    • Frequent travel is necessary and may require the use of a personal vehicle with mileage reimbursement. Occasional overnight travel will be required.
    • Collaborate with health care professionals and other state agencies to ensure appropriate services are in place to best meet the needs of individuals with special health care needs .
    • Advocate for and support individuals with special health care needs to achieve the best possible health and greatest degree of independence.
    • Connect individuals with special health care needs with resources to enhance the quality of their lives.


    Minimum Qualifications:

    • Candidates must hold a current Missouri Registered Nurse license.
    • Experience in service coordination/case management, home care/home health, and pediatrics is preferred.
    • Excellent time management, computer literacy, strong interviewing and assessment skills.
    • Must possess the ability to respect, understand, communicate with, and effectively interact with diverse populations
    • A positive attitude, exceptional organizational skills, and the ability to work well with others are strong qualifications for this role.

    • A skills test may be administered at the time of the interview.
