Social Worker III Correctional Counselor position requires a higher degree of skill in navigating the social work and evaluation of the client's needs while they are incarcerated and after their release. You will have the opportunity to be assigned to a specialized function that requires a high degree of perception, such as a special problem caseload, Home Team, Probation, Behavioral Health, HHSA, Community Resources, Emergency Response, or Medi-Cal services.
This position is responsible for difficult assignments requiring greater skill and depth of job knowledge in assessing problem situations and formulating plans for services. Qualified applicants will have knowledge of current problems and methodology in the field of public social services. This position further requires initiative, determination, detailed case management skills, and a passion to see justice-involved individuals receive the services they need to remain healthy, reformed, and successful with the end goal of reducing recidivism.
Strong candidates excel in a work environment that is fast-paced, team-oriented, and able to adapt to unforeseen circumstances.
Required skills and abilities include:- Develops and carries out social treatment plans for an assigned caseload
- Ensures all services are delivered in a supportive, culturally sensitive, and appropriate manner and in conformance with agency, state, and federal requirements
- Assists clients and family members in developing strategies to accomplish case plan goals.
Desired skills and abilities include, but are not limited to:- Conducts health risk assessments, including screening for mental health and substance use disorders.
- Identify needs and make appropriate referrals for services including health, social, community, education, employment, and/or housing.
- Provide warm linkages with designated county behavioral health and/or MCP ECM providers.
- Outline a re-entry care plan, detailed in the medical record, including release plans related to mental health, substance abuse, physical health, health-related social needs, and functional needs with input from the justice-involved individual.

Education and Experience RequiredBachelor's degree from an accredited college or university in Social Work, Marriage and Family Therapy, Counseling, Clinical Psychology, Gerontology, or related field and twelve (12) months of social work case management experience comparable to Nevada County's classification of Social Worker II.
ORMaster's degree from an accredited college or university in Social Work, Marriage and Family Therapy, Counseling, Clinical Psychology, or Gerontology or related field.
Licenses and Certificates
Possession of a valid California driver's license within 30 days of hire.
