    Job Location: This position will be domiciled at Fulton State Hospital, Ritterbusch Building. Limited overnight travel scheduled well in advance.

    Role Description

    This position will:

    • Work with Hearnes Acute Rehabilitation Program (HARP) staff, TCM entities, guardians, and service providers to ensure safe and timely discharge back to an individual’s least restrictive environment with appropriate supports.
    • Carry a short term, high support need caseload that will include individuals transitioning into and out of DMH State Operated Programs with a specific focus on HARP.
    • Collaborate with an interdisciplinary team on a routine basis.
    • Collaborate with medical staff, community agencies, and other stakeholders to coordinate comprehensive care and community placement.
    • Participate in the discharge process of individuals.
      • Ensure contact is maintained with sending service providers.
      • Ensure effective service providers are located prior to discharge.
      • Provide post-discharge support once the individual has returned to their least restrictive environment.
    • Disseminate pertinent information to stakeholders (internal and external)


    Essential Functions

    • Knowledge of DD/DMH vision, mission, and values.
    • Follow Code of State Regulations (CSRs), Departmental Operating Regulations (DORs), Division Directives and Division Guidelines.
    • Communicate verbally or in writing with individuals, providers, co-workers, and/or supervisors utilizing agency forms, computer programs and email as required.
    • Knowledge of Microsoft Office, Cimor, and Adobe Acrobat. Adaptable to updated technology and programs.
    • Read and follow directions.
    • Must travel to/from individuals' home, provider and community locations.
    • Must maintain a valid driver's license.


    Minimum Qualifications

    Bachelor’s degree or Registered Nurse license and 5 years of relevant experience [which includes, but is not limited to, fulfilling the role of Support Coordinator, Support Coordinator III, or Support Coordination Supervisor (or comparable roles within contracted TCM entities)].

    Knowledge, skills & abilities

    Demonstrates the knowledge, skills and abilities outlined in the Missouri Support Coordination Core Competencies .

    Ability to coach and mentor staff to meet the Missouri Support Coordination Core Competencies, State Plan Requirements, Medicaid Waiver Assurances, and relevant guidance related to the specialized focus areas.

    • Educate self and interdisciplinary teams about agencies, resources and supports available to individual in their community.
    • Demonstrates knowledge of the discharge process from state operated programs and facilities.
    • Knowledge of interviewing and information gathering techniques.
    • Knowledge of individual, family, and group behavior.
    • Knowledge of current social and economic challenges.
    • Ability to evaluate services and individuals' needs, functioning level, and progress.
    • Ability to understand medical and treatment records, progress notes, and other related information.
    • Ability to communicate effectively.


    Expectations

    • Coordination and Support of Complex Cases
    • Coordinate acceptance and discharge of individuals with HARP.
    • Keeps all vested parties informed about discharge progress and process.
    • Obtain, refer, or coordinate services for individuals to alleviate social, environmental, and economic challenges.
    • Attend all treatment planning sessions required for cooperative planning.
    • Communicate to individuals, families, and/or legally responsible person(s) accurate information about behavioral health diagnosis and treatment, developmental diagnosis, medication management and behavioral interventions.
    • Serve as liaison between the Department and service providers, community groups, and other state agencies.
    • Support a small, high intensity need caseload
      • Carry a short term, high support need caseload that will include individuals transitioning into and out of DMH State Operated Programs with a specific focus on HARP.
      • Collaborate with sending and receiving support coordination teams, other state agencies, and service providers to ensure continuity of care as individuals transition to new community living environments.
      • Interview individuals, family members, legally responsible person(s) and outside agencies to gather data and inform support coordination goals and outcomes.
      • Ensure your caseload documentation meets federal and state requirements.
      • Facilitate meetings as needed.
    • Serve as a specialist in statewide support coordination practices
      • Expert in support coordination practices, rules, and regulations, and identifying statewide resources.
      • Provide coaching, and necessary training and tools to the Support Coordination Teams serving those in DBH State Operated Facilities and/or DD State Operated Programs.
      • Develop and maintain effective working relationships with internal and external stakeholders.
      • Identify areas of inconsistency and problem solve with team members to develop one path forward.
      • Effectively communicate any updates to processes, policy and/or procedure.
      • Attend regular meetings with State Support Coordinators and Support Coordination Leadership.
      • Present information to stakeholder groups both in person and virtually.
      • Attend 40 hours of professional development and/or training that provides helpful resources and information for the State Support Coordination Teams.
    • Perform other duties as assigned.
