What you’ll do: - Screen referrals received via phone and online for appropriateness and completion;
- Provide information about Division services and eligibility determination process;
- Assist with completion of application paperwork;
- Prepare and submit record requests;
- Create and maintain electronic case files;
- Gather information utilizing effective interview methods;
- Prepare and send formal letters and correspondence;
- Provide information about other appropriate community resources and services;
- Input all actions and information into multiple sources, systems, and platforms;
- Complete all responsibilities following applicable Federal/State/Department/Division regulations, statutes, waiver assurances, directives, and policies;
- Attend trainings, meetings, and other events virtually and/or in-person, as requested (minimal State-wide travel necessary)

Role Description: This position performs an integral function for the Division of Developmental Disabilities’ Intake Unit. This role is responsible for providing administrative and case management support as part of the team that determines if an applicant meets eligibility criteria to be served through the Division.
