What You'll Be DoingWake County Housing is seeking a dedicated Housing - Registered Nurse to join the Cornerstone Community Support Program. The Community Support Program provides specialized support services to clients engaged with primary Permanent Housing and Support Services (PHSS) programs. The Housing- Registered Nurse will provide nursing services in a community setting to clients who have experienced homelessness and are living with chronic health and mental health conditions. This position is ideal for a passionate team-oriented nurse who enjoys working with underserved populations in a variety of settings.
Essential Functions - Provide nursing care according to professional standards including assessment and monitoring of physical and mental illness, service planning and implementation, injections, medication management, and care coordination
- Coordinate with clients, referring case managers, and medical and/or psychiatric providers to identify health needs and develop interventions to improve health outcomes
- Educate clients on health care status and diagnoses to improve health functioning and encourage self-care activities
- Connect clients to medical care by completing referrals, arranging appointments, and attending appointments with clients as needed
- Arrange and facilitate health-related educational groups
- Coordinate health events such as clinics and health fairs
- Follows Wake County Code of Conduct and adheres to strict confidentiality guidelines, including HIPAA
- Maintains required licensure for position by completing all continuing education requirements and following the standards of care/ethical code of the profession
- Provides services in the office, community, and client homes

The Basics (Required Education and Experience) - Graduation from an accredited school of Nursing

- 3 Year Mental health, psychiatry, working with special needs, chronically homeless populations
How Will We Know You're 'The One'? - Experience working with individuals who are experiencing homelessness and living with severe mental illness, substance use disorders, and complex health issues
- Ability to develop rapport and trust with clients
- Knowledge of Harm Reduction and Motivational Interviewing in the context of physical and mental health
- Ability to work as part of a multidisciplinary team
- Ability to adapt contents and teaching methods to client’s level of education, cognitive ability and learning style
- Ability to work independently and maintain professional boundaries
