Housing - Social Worker to become part of the
Housing Affordability and Community Revitalization (HACR) team! The
Housing - Social Worker works within the Homeless & Prevention Services Division - Wake Prevent and will report to the Social Worker - Supervisory at Oak City Cares. This interdisciplinary team provides case management, support, counseling and guidance to populations with special needs. This position is ideal for a passionate
Housing - Social Worker who is ready to make a direct impact in the lives of program participants in our community.
Responsibilities include:- Assessing the needs of program participants, develops individualized case plans and lead program participants to greater self-sufficiency and retention of permanent housing
- Building relationships and connections to community resources, services and partners
- Conducting home visits at least once a month or more often if necessary
- Providing appropriate resources, tools and counseling to assist residents in achieving their case plan goals
- Providing transportation as necessary
- Assisting with identifying suitable housing options based on the client’s needs
- Assisting program participants with obtaining important documents ( i.e., social security cards, birth certificates, driver’s license and award letters, etc.)
- Documents case notes and related information in the Homeless Management Information System (HMIS) and/or others systems in a timely and accurate manner

Affordable housing is also critical to preserving Wake County’s economic competitiveness by offering housing for workers at all income levels, supporting housing stability and economic opportunity for its residents and furthering Wake County’s commitment to healthy and inclusive growth. The Department’s programs include: Community Revitalization, Development Financing, Homeless and Prevention Services, Emergency Shelter, Support Services and Permanent Supportive Housing.
The Basics (Required Education and Experience) - Bachelor's degree in Social Work or related field
- One year of job-related experience
- Valid Driver's License and "safe" driving record
- Equivalent education and experience are accepted

Beyond the Basics (Preferred Education and Experience) - Master’s degree in Social Work (MSW)
- Housing/Homeless Prevention Experience
- Case management experience
- Bilingual
