    The Case Manager works closely with the HIV/STD Program, especially infectious disease patients, encouraging them to improve lifestyle choices to improve their immediate condition and potential long term health outcomes.

    Examples of Duties

    Case manages Denton County's Adult Health Clinic. Includes field record review, home visits, phone calls, emails and any combination thereof to assure a strong relationship is created with the patient. Such contacts are designed to facilitate case investigations.

    Ensures adequate documentation in the appropriate field record of the patient, including the messages delivered, the patient's receptiveness, and any addition pertinent information.

    Participates in periodic team meetings to discuss strategies used and the outcomes observed. Participates in the evaluation of the effectiveness of multiple intervention strategies for particular clients.

    Discusses client contact with staff members to assure optimal continuity of care.

    Assists in developing reports and other documents to support grant related funding.

    Coordinates and performs assistance to clients in completion of eligibility forms.

    Interviews clients for possible referrals to other social agencies.

    Maintains positive relationships with other social agencies.
    May teach and present STD/HIV educational programs within the school districts of Denton County.
    Regular and punctual attendance is required.
    Performs other related duties as required.

    Minimum Qualifications

    Bachelor's degree in related field and a minimum two years of experience in a related field required; or an equivalent combination of education and related experience required.

    Additional related education may be substituted for work experience.

    Possession of a valid driver's license. Must be able to obtain a State of Texas driver's license within 90 days of employment.
