What You'll Be Doing

Wake County EMS is expanding its Advanced Practice Paramedic program. As an industry leader in mobile integrated health, we are looking for individuals who demonstrate adaptability, problem-solving skills, strong clinical proficiency, and high levels of compassion and empathy for our most vulnerable populations.

Respond: As an Advanced Practice Paramedic, you will be responding to a wide variety of calls for on scene leadership, clinical management and assistance with patient care. These calls include high acuity conditions, mental health needs and substance use disorders.

Reduce: You will work to identify our high-volume utilizers’ barriers to care and help them navigate the healthcare system. Working with a variety of community partners, you will work to integrate the needed care to improve patient outcomes and reduce EMS utilization.

Redirect: At Wake County EMS, we believe that it is paramount to get patients to the right place, at the right time, for the right care. Our Advanced Practice Paramedics are empowered to transport mental health and substance use disorder patients to appropriate alternate care facilities to seek the specific care and treatment this populations needs.

Essential Functions:
  • Work effectively both as an individual and when needed as a team leader
  • Integrate with other emergency responders and provide advanced patient care (beyond the function of a regular Paramedic) and on scene clinical management on the scene of high acuity calls
  • Perform advanced life support skills utilizing liberal standing order protocols as well as advanced decision-making skills to guide patient care
  • Perform post-dispatch follow up for specialty patients in cooperation and collaboration with community partners and healthcare agencies
  • Apply specific criteria and protocols to provide the most appropriate care and treatment for patients who are in crisis from substance use disorders
  • Perform home visits for transitional care patients, high volume utilizers and other specialty patients to provide patient education directly related to medical conditions and follow prescribed case management as part of a community partnership to reduce hospital admission/re-admission
  • Review potential high-volume utilizers to evaluate with community partners any specialty treatment and care plans
  • Participate in meetings when required with partner agencies and provide professional feedback
  • Participate in the quality assurance process as proscribed by the Division of Medical Affairs

Participate in evidence-based medicine data entry as proscribed by the Division of Medical Affairs Chief Clinical Quality Officer
Hazmat Gear Requirements: Due to the unpredictability of the hazards encountered in the course of work, the applicant must be able to successfully fit test in, wear, and perform essential job functions in respiratory personal protective equipment including, but not limited to, NIOSH-certified N95/P100 air-purifying respirators, and a full-face tight-fitting respirator with the appropriate air-purifying cartridge. Applicant must also be able to wear, and perform essential job functions in Level C personal protective equipment (Full Tyvek suit, chemical-resistant gloves, chemical-resistant boots).

Physical Requirements: During the first week of the Wake County EMS New Hire Academy, staff must demonstrate a minimum level of physical ability and fitness by successfully completing the Wake County EMS Physical Ability Test (EMS-PAT). Incumbent employees, in any position which have field response as part of their job duties, must successfully complete the EMS-PAT annually. The purpose of this physical test is to simulate and evaluate an employee’s ability to perform the minimum physical requirements of the job. The test is performed solo (i.e., tasks that would traditionally require two employees have been modified so they can be completed by one employee), the events are performed continuously and there is a time limit of 7 minutes and 5 seconds. The events in the test consist of unbuckling a seatbelt and exiting a vehicle from the driver’s seat; stretcher lift simulation (using a 95-lb curl bar); stretcher movement with equipment 100ft (50ft down and 50ft back) over curb stops bolt to the floor; stretcher stair carry simulation (using a 95-lb curl bar); equipment lift and place; equipment movement/transfer (which includes kneeling 8 times); patient drag (using a 150-lb manikin strapped to a backboard) a total of 100ft; chest compressions (2 minutes of chest compressions at a rate of 120 a minute); repeat stretcher movement; climbing into the back of ambulance through side door.

Wake County EMS is a nationally recognized leader in EMS and prehospital medicine. The Advanced Practice Paramedic program serves Wake County residents and visitors by linking them with alternatives to traditional transport to the emergency department including mental health care access, opioid overdose intervention and falls assessment. Additionally, the Advanced Practice Paramedics respond to high acuity incidents to serve as a leader and resource for EMS crews on scene. Wake County EMS responded to over 130,000 requests for 911 EMS services in 2023. The Basics (Required Education and Experience)

  • Associate's degree or equivalent in Emergency Medical Services or related field
  • At least three years as medically cleared paramedic in a transporting 911 EMS agency
  • Valid Driver's License and "safe" driving record
  • Certification as a North Carolina Emergency Medical Technician-Paramedic (EMTP) or eligibility for such certification
  • Successful completion of the Wake County EMS System Academy and Release to Practice if not already medically cleared in the Wake County EMS System within one year
  • Successful completion of a Wake County EMS System Mobile Integrated Health Academy within one year
  • Equivalent education and experience are accepted

Beyond the Basics (Preferred Education and Experience)

  • Bachelor's in Emergency Medical Services or equivalent
  • Two plus years of experience as a Wake County Field Training Officer
