] . The annual salary for this position is up to $50,000.00 based on education and experience. Position Description
The Community Health Worker (CHW) Coordinator will provide leadership and direction to community health workers across the state through coordinating training and education activities in a meaningful way. The primary purpose of this position is to understand community needs and assist with all levels of assessment, planning, implementing, and evaluating health workers trainings and programs.. The CHW Coordinator will assure consistent community health worker standards for achieving health outcomes across the state. Duties include, but are not limited to:
•Coordinate local and state community health worker services training.
•Provide resources for community health worker learning and community needs.
•Develop innovative platforms and build a network of community health workers.
•Develop and disseminate community health worker education materials for all to access.
•Coordinates activities and efforts by conducting research, maintaining a statewide repository of tools and products.
•Interact with and provide technical assistance to internal customers (such as state and county health department's community health workers); as well as external stakeholders from the public, private, and nonprofit sectors.
•Research and identify evidence-based practices, national best practices, promising development ideas, and similar health driven practices.
•Facilitate meetings and assist with project planning for the development, implementation, and evaluation of health improvement plans, systems, and initiatives. Other Duties

Work is typically performed in an office setting with a climate-controlled settings and exposure to moderate noise level. While performing the duties of the job, employee is required to talk, stand, walk, and reach with hands and arms. This position requires long period of sitting and daily use of computer and phone. MINIMUM QUALIFICATIONS
The requirements at this level consist of a bachelor's degree in Health Education, Health Promotion, Community Health, School Health Education, Health Studies, Nutrition, Health Behavior, or Nursing. Plus 2 years of experience in public health education or community development OR a master's degree in Public Health, Health Education, Health Promotion, Community Health, School Health Education, Health Studies, Nutrition, Health Behavior, or Nursing.NOTES REQUIREMENTS:
