Arrowhead Regional Medical Center (ARMC) is recruiting for Contract RN Care Managers who, in collaboration with the healthcare team, perform needs assessment on chronically ill and other patients, develop and help implement plans of care, provide patient care and education to ensure plan adherence in the ambulatory care setting. Positions manage patients with complex health care needs who may be at risk for readmission, proactively outreach to high-risk patients and develop initiatives that support patient self-management of chronic diseases and care coordination.

Whole Person Care Program
The Whole Person Care Program is a partially grant-funded program with continued funding through June 8, 2023. The program's funding is contingent upon the availability of funds and can be terminated or amended at any time by the Director or his or her designee. The Whole Person Care Program has developed and instituted a WPC Engagement Team to improve patients' health and well-being through coordinated efforts to address physical health, behavioral health, and social services, resulting in increased access and utilization of County resources.

Appointment: This is a contract position which does not obtain status as a regular County employee. Working as a contract employee does not preclude you from applying and competing for Regular positions within the County (Refer to the County of San Bernardino website for current open recruitments).

Background: Applicant must successfully pass a background check prior to appointment.

Work schedule: The work schedule is approximately 40 hours per week.

Travel: Travel throughout the County may be required. At the time of hire, a valid California Class C driver license and proof of automobile liability insurance is required for the individual providing the transportation. Mileage reimbursement is available.

Minimum Requirements

Candidates must possess the following:

Must possess and maintain a current license as a Registered Nurse with the State of California Board of Registered Nursing.

Certification: Must possess and maintain current Healthcare Provider level Basic Life Support (BLS) certification issued by the American Heart Association.


One of the following:

Option #1
Three (3) years registered nurse experience in an acute care hospital.

Option #2
Two (2) years of experience in Primary Care, Outpatient Care, Care Management or Community Health.

Desired Qualifications

Candidates possessing a Bachelor's or Master's degree in nursing, healthcare administration or related field are highly desired.
