who, in collaboration with the healthcare team, perform needs assessment on chronically ill and other patients, develop and help implement plans of care, provide patient care and education to ensure plan adherence in the ambulatory care setting. Positions manage patients with complex health care needs who may be at risk for readmission, proactively outreach to high-risk patients and develop initiatives that support patient self-management of chronic diseases and care coordination.

Whole Person Care Program
The Whole Person Care Program is a partially grant-funded program with continued funding through June 8, 2023.
