who, in collaboration with the healthcare team, perform needs assessment on chronically ill and other patients, develop and help implement plans of care, provide patient care and education to ensure plan adherence in the ambulatory care setting.Positionsmanage patients with complex health care needs who may be at risk for readmission, proactively outreach to high-risk patients and develop initiatives that support patient self-management of chronic diseases and care coordination.

This is a contract position which does not obtain status as a regular County employee. Working as a contract employeedoes not preclude you from applying and competing for Regular positions within the County (Refer to the County of San Bernardino website for current openrecruitments).Work schedule:
The work schedule is approximately 40 hours per week.Travel:
Travel throughout the County may be required.At the time of hire, a valid California Class C driver license and proof of automobile liability insurance is required for the individual providing the transportation. Mileage reimbursement is available.Minimum RequirementsCandidates must possess the following:
Must possess and maintain a current license as a Registered Nurse with the State of California Board of Registered Nursing.Certification:
Must possess and maintain current Healthcare Provider level Basic Life Support (BLS) certificationissued by the American Heart Association.AND
One of the following:
Three (3) years registered nurse experience in an acute care hospital.Option #2
Two (2) years of experience in Primary Care, Outpatient Care, Care Management or Community Health.
An equivalent combination of education and experience may be considered. Desired Qualifications
Candidates possessing a Bachelor's or Master's degree are desired.Selection Process
