Job Description

Social Workers are licensed professionals who provide case management services to children, families, the elderly, and other individuals within local communities and institutions. Incumbents possess a degree of knowledge and proficiency sufficient to perform work independently with little or no additional training. This position is located within the Department of Health and Human Services, Aging and Disability Services Division (ADSD) - Community Based Care in Reno, NV. ADSD provides an atmosphere of innovative thinking, teamwork, friendly co-workers, and provides a positive working environment. The mission of ADSD is to provide leadership and advocacy in the planning, development and delivery of a high quality, comprehensive support service system across the lifespan to allow all of Nevada's elders, children and adults with disabilities or special health care needs, to live independent, meaningful and dignified lives to the greatest extent possible. This position will be primarily responsible for providing case management for recipients in the Home and Community Based Waiver (HCBW) program for the Frail Elderly (FE) and Persons with Physical Disabilities (PD). This includes interviewing and assessing individuals to determine if the criteria are met for placement on the HCBW and providing case management through other Medicaid waivers, and State Funded programs such as Personal Assistance Services (PAS), Community Options Program for the Elderly (COPE) programs and Homemaker Program. This position will complete monthly contacts with program recipients, complete required reports to appropriate agencies, complete documentation in set databases and complete community program referrals as needed.
