About the Role: This position is responsible for managing the daily operations related to nursing care and CMAs in line with clinical practice standards. This position is a working RN role and will spend 20% of time in administrative duties and 80% in the care coordinator role. Location: 224 West Clarendon Drive, Round Lake Beach IL Hours: Monday - Friday (8:00am - 4:00pm) FLSA STATUS: Exempt Skills and Experience Oversees clinical nursing care, ensuring compliance with established regulations and standards including quality assurance, confidentiality, and medical record documentation. Provides direct supervision to nurses and subordinate staff, ensuring appropriate assessment and treatment of patient health. Serves as key problem-solving resource for staff on patient issues. Studies patient needs/trends, analyzes patient flow problems, and suggests solutions to patient care problems. Collaborates with the practice manager, and lead MD on development and implementation of nursing department’s mission, goals, policies, procedures, budget quality assurance plan, and work standards. Compiles/analyzes data on patient outcomes, quality assurance results, budget/goal achievement, and performance/productivity and prepares reports for the practice manager and the clinical administrative team. Participates in medical practice planning related to clinic and emergency/disaster response system. Ensures that offices/departments are clinically staffed appropriately. Collaborates with human resources department on recruiting, selecting, orienting/training, and evaluating nursing staff. Provides patient care, including administering medications, according to Provider’s orders within their scope of practice, established standards and administrative policies of the department, as necessary. Responsible for ensuring Nursing staff and CMAs are up to date with licensure and yearly training in collaboration with the clinical compliance team. Participates in audit readiness and ensures staff is prepared for audits. Ensures that clinical supplies including vaccinations are organized and up to date. Performs other duties as assigned Maintains required training, licensure, and/or certification. Maintains confidentiality of privileged information and adheres to patient privacy laws. Demonstrates culturally competent healthcare delivery and fosters same among all staff. All other relevant duties as assigned. Education, Qualifications, and Certification: Current Illinois RN license Minimum five years of professional nursing experience including two years’ clinical supervisory experience, preferably in a medical practice setting. For internal candidates – 5 years of nursing experience. Clinical supervisory experience requirements may be off set by performance, skills and potential demonstrated during your time as an employee. About the Role: Under the supervision of Nutrition Services Assistant Coordinator, supervises WIC staff and services at assigned site. Assists staff in the provision of direct nutrition services for WIC/FQHC clients in accordance with program and agency requirements. Work involves direct client contact and is performed in accordance with state and local agency policies and procedures. Location: 224 Clarendon Road, Round Lake Beach IL Hours: Monday, Wednesday, Thursday, and Friday (8:00am - 4:30pm) Tuesday (10:30am - 7:00pm) FLSA STATUS: Exempt Skills and Experience Supervises staff and daily clinic activities of WIC services at assigned site to achieve caseload, client visit, and program goals. Provides certification, nutrition counseling using motivational interviewing, medical nutrition therapy (including Gestational Diabetes assessment and counseling), breastfeeding support, and appropriate referral for WIC/FQHC clients according to established criteria. Documents clinical information including nutrition care plans in clients’ electronic health record (Cornerstone and NextGen as appropriate) according to agency and state standards. Assists in implementation and monitoring of protocols, policies and procedures to ensure services meet program/grant objectives and are in compliance with Federal/State requirements. Participates in ongoing program evaluation including monitoring of program/grant performance measures and key performance indicators, and implementation of Continuous Quality Improvement activities to improve program and health outcomes for clients/patients. Performs monthly competency assessments and annual performance appraisals for assigned staff and provides timely feedback in meeting established performance goals. Assists in integration of Nutrition Services with other LCHD Programs. Prepares reports on staff activities, client data, and service delivery as required. Education, Qualifications, and Certification: Requires a Bachelor’s Degree in Nutrition related field. Minimum of two years clinical/community experience (one year if Master’s Degree). Requires current Illinois licensure as a Licensed Dietitian Nutritionist or Illinois licensure by reciprocity within six months of initial date of employment. Requires current Registered Dietitian/Registered Dietitian Nutritionist (RD/RDN) credentialing by the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics. Provides medical services for patients on a part-time or full-time basis as described in their contract, at one or more of the clinical facilities of the Service Area. Is knowledgeable of and is in compliance with CLIA, OSHA, and JCAHO regulations for safety, infection control, equipment operation, and other applicable areas. Upon request, provide teaching, supervising and/or precepting for students and residents as needed in support of Community Health Center objectives and in accordance with requirements set forth in agreements with educational institutions. Education, Qualifications, and Certification: Requires graduation from an accredited school of medicine, the completion of one year of residency (internship) in an approved program, and medical license in the State of Illinois. A completed residency in an approved program in a relevant specialty, with Board eligibility is preferred. Post residency experience in Primary Care is preferred.
