Conducts an assessment of clients' needs using a variety of screening tools to identify issues such as mobility problems, depression, and needs related to the social determinants of health, etc. Connects patients with appropriate clinical staff when there are questions about their treatment plans or health care recommendations. Identifies, anticipates and assists clients with community, cultural, social and educational barriers within the healthcare system. Assists patients in removing barriers to care and individual self-management goals by encouraging self-sufficiency and advocacy. Models appropriate strategies for self-help and coping skills with effective communication. Provides peer health education through home visits and/or clinic visits to assess clients' needs and concerns related to their health, family, and community. Provides educational materials for patients. Documents interventions and maintains accurate records. Builds and maintains relationships with clients, support networks and community agencies.
