DESCRIPTIONThe City of Long Beach, Department of Health and Human Services is recruiting for one full-time, unclassified, at-will In-Home Outreach Counselor (Counselor II) in the Community Impact Division. Reporting to a Public Health Professional II-III (Clinical Supervisor), this position will provide in-home outreach counseling to referred families to promote strengthening family function, reduce out-of-home placement, and provide case management, education, and linkages to services. This position is located at the Ron Arias Health Equity Center 6335 Myrtle Ave Long Beach, CA 90805.
EXAMPLES OF DUTIES- Provide in-home case management visits to participating families a minimum of one time per week, or as determined by the service plan;
- Attend Multidisciplinary Case Planning Committee (MCPC) meetings and contribute to the development and monitoring of each individualized service plan for each family;
- Observe children and assess home environment for child safety risk factors;
- Provide referrals and linkages to parenting training, youth services, mental health services, housing services, job training, transportation, and other needed services as identified in the service plan;
- Identify community resources and pursue new referral contacts to meet client needs;
- Monitor and perform follow-up on participation in activities and services defined in the service plan;
- Provide crisis intervention services including suicide risk assessment;
- Serve as the liaison and advocate for families participating in the program;
- Maintain case records and required reporting documentation, including data collection forms, progress notes, assessments, monthly reports, service logs and referral forms;
- Assist with events, activities, and services of other Center for Families and Youth programs, as required;
- May facilitate MCPC meetings; and,
- Perform other related duties as required.
REQUIREMENTS TO FILE- High School Diploma or equivalent certification.
- Four years of professional experience working in a community-based program as a case manager or working in any capacity with in-home services.
- Academic degrees in Social Work, Public Health, Social Services, Psychology or a closely related field may be substituted for required experience on a year-for-year basis. (Proof of education is required.*)
- Valid motor vehicle operator license.
- Required to obtain a California motor vehicle operator license within 6 months of employment if out of state. A DMV K4 Driver License Information Report is required prior to appointment for California residents.
DESIRABLE QUALIFICATIONS: - Bachelor's or Master’s Degree in Social Work, Public Health, Social Services, Psychology or a closely related field.
- Bilingual skills ( Khmer, Spanish, Tagalog).

- Ability to provide crisis intervention and assessment with high risk and special needs populations.
- Ability to work effectively with a wide variety of people and populations by consistently exercising tact, good judgment, and a proactive, problem-solving focused style.
- Ability to deescalate and resolve conflict effectively.
- Ability and willingness to provide therapy, if qualified, to clients as needed.
- Effective interpersonal, written, and verbal communication skills.
- Proficiency in the use of computers, including email, internet, and office productivity software (e.g. Microsoft Word, Excel, etc.)
