If you're passionate about making a real difference in the lives of individuals with intellectual and developmental disabilities, we'd love to hear from you.RequirementsMaster's degree in a human services field + 1 year of IDD experience, or Bachelor's degree in a human services field + 2 years of IDD experience (per PA 6400.44)Valid PA clearances: FBI fingerprint, PA State Police background check, and Child Abuse History (Act 151)Strong written and verbal communication skills and comfort with electronic documentation systems (HCSIS, Therap, or similar)Valid driver's license, reliable transportation, and ability to travel regularly between residential sitesCPR/First Aid certification required within 90 days of hirePreferred: PA 6400/ODP waiver experience, behavior support planning background, and bilingual Spanish ability (Philadelphia market).SummaryJob Duties:Develop, implement, and monitor Individual Support Plans (ISPs) rooted in person-centered planning principlesFacilitate ISP team meetings with individuals, families, Support Coordinators, and providersEnsure compliance with PA Chapter 6400 regulations and maintain audit-ready documentationCoordinate with nursing staff on health monitoring and medical care for individuals with Medically Complex Conditions (MCC)Train and mentor Direct Support Professionals on ISP implementation, data collection, and person-centered practicesConduct site visits, review documentation, and support licensing inspections and quality improvement efforts#J-18808-Ljbffr
