The Department of corrections is seeking a highly motivated and qualified individual for a project position as a Corrections Specialist 3 providing case management services for facilities on the Eastside of Washington State.
This position supports the DOC mission of maintaining safe communities and positively changing lives by supporting clients living with HIV who are releasing from incarceration to community or partial confinement. This position will serve as a program specialist for the Health Services Division providing critical services funded by the Department of Health (DOH) contract funding, to support individuals living with HIV for transition and integration into the community in the DOH specific service area. This position will conduct senior-level work providing comprehensive case management services to justice impacted individuals. Providing linkage with healthcare and health related social needs reduces recidivism, improves client healthcare outcomes, and supports people's success post release from incarceration in their specific service area. This position will coordinate and implement activities to ensure support related to continuity of care from prison to community for specific counties covered by contract. This position will ensure support for transition from incarceration and linkage to specific counties and community resources/ services. Work performed will be responsive to the duration of the DOH contract and any other funder requests to ensure compliance with federal requirements. This grant-funded position is expected to last no more than 12 months and can be ended at any time prior to 24 months, due to funding.

Duties
Provides services for justice impacted individuals living with HIW per DOH/contract requirements

  • Provides community linkage Non-Medical Case Management (NMCM) services pre-release via in person visits, Teams meetings or Telehealth appointments to assess client needs and coordinate care from prison to community to support transition from incarceration to community within DOH counties.
  • Coordinate with Prison Reentry staff any client concerns, ability to meet medical needs in the county of release, housing plans and needs, release plan goals, and any other support needs that will aid in a smooth transition into the community.
  • Make initial medical appointments for clients HIV care and medical case management services with new or re-establishing with prior providers. Confirm medical insurance coverage approval and assist if needed to change medical insurance coverage to align with clinic/provider accepted medical insurance.
  • Assist as needed, eligible clients in obtaining access to other public and private programs such as Medicaid, Medicare, Managed Care Organization services, pharmacy assistance programs, other state or local health care and supportive services.
  • Completes initial and ongoing assessments of service needs, develops comprehensive, individualized care plan, advocates for clients when needed, continuously monitors plan, and updates plan as required by the DOH contract.
  • Review records and custody database to determine corrections stipulations and other information as needed.
  • Facilitates multidisciplinary meetings with facility Health Services Reentry Team (e.g. Social Worker, Reentry Nurse), clinical team (Infection Prevention team members), client, family/support, community provider, and other partners as needed to ensure smooth continuity of care.
  • Facilitate warm hand off to community healthcare clinic pre-release if clinic is amenable.
  • Collaborate with facility-based Health Services Reentry team (Social Workers/Reentry Nurses, etc.) on care coordination and to complete Health Services Reentry Plan form.
  • Conduct in person, telephone/zoom contacts with eligible clients, post release -enrolled in CLCC program per the DOH contract/grant requirements.
  • Coordination, guidance, and assistance to program clients in accessing housing, medical, social, community, legal, financial, employment, vocation, and/or other needed services.
  • Make appropriate referrals to community agencies including HIV prevention, community agencies, mental health, and substance use treatment as needed to support the client and promote self-sufficiency.
  • Facilitate transportation to first medical appointment. Based on client need and as budget allows, facilitate transportation to appointments for Social Security, DSHS, and housing.
  • Staff caseload with supervisor and document as case conferencing in Provide Enterprises.
  • Facilitate and provide food vouchers to release individuals or individuals in need as budget allows.
  • Provide housing referral services including assessment, search, placement and housing advocacy services on behalf of the eligible client as well as fees associated with these activities.
  • Support clients housed with DOH contract funding for up to 24 months post-release including non-medical case management, facilitating housing payments, being the liaison to property staff, partnering with community Medical Case Manager, and other supporting agency staff on client housing stability, facilitating rent payments, and supporting eviction prevention.
  • Coordinate linkage to medical, community medical case manager, mental health, primary care, and/or other healthcare appointments in the community post-release.
  • Review records and custody database to determine community corrections stipulation and other information.
  • Build and maintain partnerships with internal and external stakeholders with a focus on housing and medical stability.
  • Maintains an up-to-date caseload list with tracking to indicate each step has been completed per DOH contract requirements/protocol in identified team file location.
  • Coordinate with DOH HIV Peer Reentry Coordinator and other DOH staff to include case consolation and team approach to coordination of reentry care.
  • Identify high risk behaviors to provide HIV-prevention counseling to clients.
  • Document all services and NMCM activity, pre/post release provided per DOC guidelines, policy and grant and/or contract requirements to include Provide Enterprises and OMNI.
  • Complete and maintain accurate, complete and current records/files.
  • Completes all work timely as outline in grant/contract requirements, DOC policy, guidelines, and protocols.
  • Track and record post-release follow-up of incarcerated individuals in community.
  • Enter Services: Food Gift Cards, Medical Transportation and housing in Provide database to include the cost paid for the service. For housing indicate if it is daily, weekly or monthly for each client through the duration of their engagement with the program.
Community Resources for rural or resource poor communities in DOH contract areas.
  • Engage with community agencies to assist with client engagement and access to services.
  • Understand and participate in community meetings, events, etc. to identify and create collaborative relationships with community resources.
  • Establish professional relationships with landlords and other community housing providers to create new housing resources.
  • Engage with other Health Services Reentry staff to share and collaborate when community resources are needed and/or identified.
  • Establish connections with DOH resources, housing resources, and landlords.
Administrative
  • Participate in monthly budget meetings. Participate in all CLCC program meetings and bi-weekly check-in with prison HIV medical providers to discuss releases happening in the next 90 days.
  • Track monthly housing payments and work with supervisor to ensure funding is available throughout the year. Update CLCC Client tracker, documenting completion of needed paperwork, reviews and updates.
  • sist with development and engagement of other funding sources.
  • Participate as a member of Quality Management team and conduct identified work to reach QM goals.
Other duties as assigned
  • Participate in Health Services projects and quality assurance as assigned.
  • Attend conferences and other education opportunities related to HIV/AIDS education, reentry from incarceration and other related topics as budget allows.

Qualifications
Required qualifications:
  • 4 years of professional experience providing healthcare case management services in adult or juvenile corrections, social services, or closely related field 2 years working with Medicaid systems. supervisory Experience.
  • Proficiency using Microsoft Office software (Word, Excel, Outlook). Excellent written and verbal communication skills
  • Note: Additional qualifying experience will substitute, year for year, for up to two years of the desired education.
Preferred/Desired Qualifications:
  • Bachelor's degree in public health, nursing, social services, human services, behavioral health services or closely allied field.
AND
  • Four years of professional experience in adult or juvenile corrections, social services or closely related field.
  • Note: Additional qualifying experience will substitute, year for year, for up to two years of the desired education. A

  • Working knowledge of HIV care continuum and care system, ideally in Washington State.

Holidays
Full-time and part-time employees are entitled to paid holidays and one paid personal holiday per calendar year.

Full-time employees who work full monthly schedules qualify for holiday compensation if they are employed before the holiday and are in pay status for at least 80 nonovertime hours during the month of the holiday; or for the entire work shift preceding the holiday.

Sick Leave
Full-time employees earn eight hours of sick leave per month. Overtime exempt employees who are in pay status for less than 80 hours per month do not earn a monthly accrual of sick leave.

Vacation (Annual Leave)

  • Coordinate with DOH HIV Peer Reentry Coordinator and other DOH staff to include case consolation and team approach to coordination of reentry care.
  • Identify high risk behaviors to provide HIV-prevention counseling to clients.
  • Document all services and NMCM activity, pre/post release provided per DOC guidelines, policy and grant and/or contract requirements to include Provide Enterprises and OMNI.
  • Complete and maintain accurate, complete and current records/files.
  • Completes all work timely as outline in grant/contract requirements, DOC policy, guidelines, and protocols.
  • Track and record post-release follow-up of incarcerated individuals in community.
  • Enter Services: Food Gift Cards, Medical Transportation and housing in Provide database to include the cost paid for the service. For housing indicate if it is daily, weekly or monthly for each client through the duration of their engagement with the program.
Community Resources for rural or resource poor communities in DOH contract areas.
  • Engage with community agencies to assist with client engagement and access to services.
  • Understand and participate in community meetings, events, etc. to identify and create collaborative relationships with community resources.
  • Establish professional relationships with landlords and other community housing providers to create new housing resources.
  • Engage with other Health Services Reentry staff to share and collaborate when community resources are needed and/or identified.
  • Establish connections with DOH resources, housing resources, and landlords.
Administrative
  • Participate in monthly budget meetings. Participate in all CLCC program meetings and bi-weekly check-in with prison HIV medical providers to discuss releases happening in the next 90 days.
  • Track monthly housing payments and work with supervisor to ensure funding is available throughout the year. Update CLCC Client tracker, documenting completion of needed paperwork, reviews and updates.
  • sist with development and engagement of other funding sources.
  • Participate as a member of Quality Management team and conduct identified work to reach QM goals.
Other duties as assigned
  • Participate in Health Services projects and quality assurance as assigned.
  • Attend conferences and other education opportunities related to HIV/AIDS education, reentry from incarceration and other related topics as budget allows.
  • Attend community meetings and workgroups as requested/needed.

Qualifications
Required qualifications:
  • 4 years of professional experience providing healthcare case management services in adult or juvenile corrections, social services, or closely related field 2 years working with Medicaid systems. supervisory Experience.
  • Proficiency using Microsoft Office software (Word, Excel, Outlook). Excellent written and verbal communication skills
  • Note: Additional qualifying experience will substitute, year for year, for up to two years of the desired education.
Preferred/Desired Qualifications:
  • Bachelor's degree in public health, nursing, social services, human services, behavioral health services or closely allied field.
AND
  • Four years of professional experience in adult or juvenile corrections, social services or closely related field.
  • Note: Additional qualifying experience will substitute, year for year, for up to two years of the desired education. A

  • Working knowledge of HIV care continuum and care system, ideally in Washington State.

Insurance Benefits
Employees and their families are covered by medical (including vision), dental and basic life insurance. There are multiple medical plans with affordable monthly premiums that offer coverage throughout the state.

Staff are eligible to enroll each year in a medical flexible spending account which enables them to use tax-deferred dollars toward their health care expenses. Employees are also covered by basic life and long-term disability insurance, with the option to purchase additional coverage amounts.

Retirement and Deferred Compensation
State Employees are members of the Washington Public Employees' Retirement System (PERS). New employees have the option of two employer contributed retirement programs. For additional information, check out the Department of Retirement Systems' web site.

Employees also have the ability to participate in the Deferred Compensation Program (DCP). This is a supplemental retirement savings program (similar to an IRA) that allows you control over the amount of pre-tax salary dollars you defer as well as the flexibility to choose between multiple investment options.

Social Security
All state employees are covered by the federal Social Security and Medicare systems. The state and the employee pay an equal amount into the system.

Public Service Loan Forgiveness
If you are employed by a government or not-for-profit organization, and meet the qualifying criteria, you may be eligible to receive student loan forgiveness under the Public Service Loan Forgiveness Program.

Holidays
Full-time and part-time employees are entitled to paid holidays and one paid personal holiday per calendar year.

Note: Employees who are members of certain Unions may be entitled to additional personal leave day(s), please refer to position specific Collective Bargaining Agreements for more information.

Full-time employees who work full monthly schedules qualify for holiday compensation if they are employed before the holiday and are in pay status for at least 80 nonovertime hours during the month of the holiday; or for the entire work shift preceding the holiday.

Part-time employees who are in pay status during the month of the holiday qualify for the holiday on a pro-rata basis. Compensation for holidays (including personal holiday) will be proportionate to the number of hours in pay status in the month to that required for full-time employment, excluding all holiday hours. Pay status includes hours worked and time on paid leave.

Sick Leave
Full-time employees earn eight hours of sick leave per month. Overtime eligible employees who are in pay status for less than 80 hours per month, earn a monthly proportionate to the number of hours in pay status, in the month to that required for full-time employment. Overtime exempt employees who are in pay status for less than 80 hours per month do not earn a monthly accrual of sick leave.

Sick leave accruals for part-time employees will be proportionate to the number of hours in pay status, in the month to that required for full-time employment. Pay status includes hours worked, time on paid leave and paid holiday.

Vacation (Annual Leave)
Full-time employees accrue vacation leave at the rates specified in (1) or the applicable collective bargaining agreement (CBA). Full-time employees who are in pay status for less than 80 nonovertime hours in a month do not earn a monthly accrual of vacation leave.

Part-time employees accrue vacation leave hours in accordance with (1) or the applicable collective bargaining agreement (CBA) on a pro rata basis. Vacation leave accrual will be proportionate to the number of hours in pay status, in the month to that required for full-time employment.

Pay status includes hours worked, time on paid leave and paid holiday.

As provided in , an employer may authorize a lump-sum accrual of vacation leave or accelerate the vacation leave accrual rate to support the recruitment and/or retention of a candidate or employee for a Washington Management Service position. Vacation leave accrual rates may only be accelerated using the rates established WAC 357-31-165.

Note: Most agencies follow the civil service rules covering leave and holidays for exempt employees even though there is no requirement for them to do so. However, agencies are required to adhere to the applicable RCWs pertaining holidays and leave.

Military Leave
Washington State supports members of the armed forces with 21 days paid military leave per year.

Bereavement Leave
Most employees whose family member or household member dies, or for loss of pregnancy, are entitled to five (5) days of paid bereavement leave. In addition, the employer may approve other available leave types for the purpose of bereavement leave.
