Job Title
Responsible for individualized client services provided in a community/residential setting. Duties include assessing, planning, implementing, monitoring, and evaluating client behaviors and outcomes while ensuring appropriate level/type of services are provided.
Position Description
- With close supervision; conducts interviews and administers assessments and screening instruments
- Maintain caseload and productivity by ensuring regular contact with clients to monitor and assess progress
- Write service plans, review progress and determine updates needed in collaboration with client
- Utilize core correctional practices while assisting clients in effective community re-integration
- Deliver ongoing services to clients which may include crisis intervention, advocacy, medication monitoring, transportation assistance, financial budgeting, housing assistance, and referral and linkage to community resources as needed
- Ensure compliance with applicable standards and agency policies
- Document all clinical activities to meet specific program, funder and regulatory criteria timely and with accuracy
- Participate in team meetings and assist in making recommendations for client status changes
- Lead educational groups and/or facilitate group curriculum
- May require transportation of clients
- Adhere to professional standards as outlined by protocols, rules, and regulations
- Other duties as assigned
Required Knowledge, Skills and Abilities
- Ability to learn case management principles and core correctional practices
- Excellent organizational and time management skills
- Effective verbal and written communication skills
- Demonstrate strong ethical and professional behavior
- Ability to relate to and interact with a wide variety of people in an ethical, culturally sensitive and respectful manner
- Strong problem solving skills and ability to multi-task while prioritizing
- Ability to utilize technology including electronic service documentation system
Job Requirements
- Bachelor's Degree in Behavioral Science, Criminal Justice or related field or; equivalent work experience as allowed/required by regulatory bodies
- May require CDCA or other specialty certification
- Valid Driver License and auto insurance
Essential Functions/Physical Demands
- Positional: Driving in accordance with job duties assigned. Frequent standing and walking. Frequent sitting.
- Gross Mobility: Rare climbing, or crawling. Infrequent balancing, stooping, kneeling, or crouching. Frequent to constant reaching and handling.
- Sensory: Rare tasting/smelling. Infrequent use of color vision. Occasional use of far visual acuity, depth perception, and field of vision. Frequent use of near and midrange visual acuity, and visual accommodation. Frequent to constant talking.
