    ABOUT THE POSITION

    The Senior Support Service Worker performs complex and highly sensitive paraprofessional work in support of the Health Care Program for Children in Foster Care (HCPCFC), with guidance provided by the Senior Public Health Nurse. This position plays a critical role in supporting children and youth in out-of-home placement, including justice involved youth, access Medi-Cal, navigate the health care system and effectively utilize Medi-Cal benefits. It also supports the County’s CalAIM and Enhanced Care Management (ECM) initiatives through inter-departmental coordination and community outreach.

    The Senior Support Service Worker supports care coordination activities, assists foster youth and caregivers in navigating Medi-Cal and managed care systems, monitors compliance with state and county health mandates, and maintains accurate health documentation, including the Health Education Passport (HEP). The role requires close collaboration with Public Health Nurses, Child Welfare Workers, Probation Officers, county departments, community-based organizations, healthcare providers, and caregivers to ensure continuity of care, particularly during placement changes and transitions. The position also supports program development by delivering trainings, tracking data, monitoring performance measures and ensuring compliance with state required reporting, while maintaining strict confidentiality in compliance with HIPAA and County policies.

    HYBRID SCHEDULE
    The Health and Human Services Department has embraced a hybrid work schedule that considers that some duties of this position may be performed remotely. However, the successful candidate must be able to report to County facilities for meetings, work assignments, and/or disaster service work upon short notice, as needed and deemed appropriate. During the initial probationary period, the successful candidate may be required to work primarily onsite before establishing a hybrid work schedule.

    OUR HIGHLY QUALIFIED CANDIDATE
    Our highly qualified candidate is an experienced, highly organized, and compassionate professional with a strong background in health, social services, or community-based programs serving vulnerable populations. They bring demonstrated experience supporting high-need populations navigate complex public systems, such as hospital systems, Federally Qualified Health Centers (FQHCs), Medi-Cal Managed Care Plans, and/or state and county government. They possess a working knowledge of child welfare, juvenile justice programs and trauma-informed approaches to care.

    The highly qualified candidate excels at coordination and problem-solving across multiple systems and settings, including public health, child welfare, juvenile probation, behavioral health, and community partners. They are technologically proficient, comfortable tracking performance measures and maintaining detailed records, and able to support evolving CalAIM and ECM initiatives with flexibility and resourcefulness.

    Equally important, the highly qualified candidate demonstrates cultural humility, discretion, and professionalism. They communicate clearly and respectfully, manage sensitive information with care, and are adept at balancing competing priorities in a fast-paced, high-volume environment. A commitment to equity, collaboration, and improving health outcomes for children and youth in foster care is essential to success in this role.

    QUALIFICATIONS
    Knowledge of:
    • The ethnic, economic, and social factors affecting residents and special populations of the community or clients served by Health and Human Services programs.
    • Terminology and practices of providing paraprofessional support in a medical office.
    • Universal Precautions and clinical protocol as it applies to collecting, cataloging and processing laboratory specimens and preparing for transport.
    • Basic medical terminology and case management concepts.
    • Major community resources including social, employment, and health services.
    • Ethnic, economic and social barriers which inhibit residents from seeking and receiving services and methods of overcoming or lowering service providing barriers.
    • Acceptable household management and personal care standards such as basic nutrition, economical meal planning, money management and budgeting and personal health and hygiene standards, methods and practices.
    • Normal health needs of infants, children, adolescents, adults, and aged persons.
    • Signs and symptoms requiring medical assistance.
    • Interviewing and information gathering techniques.
    • Computer applications related to the work.
    • Standard office practices and procedures, including filing and the operation of standard office equipment.
    Ability to:
    • Evaluate client needs through interviewing and observation.
    • Perform a variety of sensitive and complex paraprofessional support work.
    • Resolve conflicts between clients and providers.
    • Recognize specific indications of need for various services and develop service delivery plans.
    • Maintain accurate records and files.
    • Perform and follow clinical protocols accurately.
    • Make accurate complex arithmetic calculations.
    • Prepare reports related to the work performed.
    • Establish and maintain effective working relationships with co-workers, clients, children, representatives of the community served, employees of County departments and other agencies and the public.

    MINIMUM QUALIFICATIONS
    Four years of experience in a health, social service or community organization including one year as a Support Service Worker II or the equivalent which demonstrates possession of the stated knowledge and abilities and twelve (12) semester units of education in health science, behavioral science and/or social welfare from a college or university.

    College-level education in a field as listed above may be substituted for the experience on a year-for-year basis to a maximum of two years. An additional year of experience may substitute for the educational requirement.
