    Job Description

    Job Location: This position will be located at 8800 E. 63 rd St, 6 th Floor, Raytown, MO 64133

    This position will serve as a Facilitator for the Bureau of Special Health Care Needs (SHCN) Healthy Children and Youth and Medically Fragile Adult Waiver Programs. SHCN is looking for a team player with a keen eye for detail who wants to make a positive impact for individuals with special health care needs and their families. We offer a work culture that promotes a work-life balance in a supportive environment that values both teamwork and autonomy. Work hours are 8 am - 5 pm, Monday- Friday, flex schedules may be considered.

    • Review documents for accuracy, completeness, and compliance with established policies and procedures.
    • Maintain records for the implementation and management of the program, gather and provide data, prepare reports relative to program activities.
    • Provide training, guidance, and technical assistance, to program partners and SHCN staff.
    • Use word processing, spreadsheets, databases, and other program related computer applications necessary to meet program goals
    • Assist with intake of new referrals, assist with determining participant eligibility for program services, program enrollment, and initiate the process to assess for services.


    Minimum Qualifications:

    • Bachelor’s degree. (Applicable work experience will be allowed as a substitution).
    • Attention for detail, excellent problem solving skills, and the ability to work closely and collaboratively with SHCN staff, health care providers, and others involved in program implementation.
    • Excellent communication skills; be able to explain, demonstrate, and train program guidelines and technical processes.
    • Work independently, multitask, and demonstrate superior initiative. Positive attitude, exceptional organizational skills, and the ability to work well with others.
