    Nursing Process: Nurses who provide patient-centered care at Bristol Hospital and affiliates are the coordinators of care and provide continuity of care across the continuum. Nurses consider patient's unique need and individualize care accordingly and establish goals towards desired outcomes. Nurses practice autonomously and consistent with professional standards; they assess and provide appropriate patient care based on competence, expertise and knowledge.

    • Independently uses and documents the nursing process to plan, deliver, and evaluate goal focused, individualized, safe, population-specific care for all patients including those with complex pathophysiological needs and/or psychosocial needs.
    • Assure coordination of thorough and complete, population-specific continuum of care planning for all patients.
    • Independently accesses health care team and provides appropriate information for complex continuum of care planning.
    • Identifies patient problems, and unit problems, when goals are not met and utilizes problem solving skills to participate in finding solutions to resolve problems and follows through to implement solutions.


    Nursing Leadership: Nurses advocate for self, peers and patients

    • Demonstrates accountability as an individual and team member in meeting basic patient/family needs.
    • Using appropriate delegation skills, manages clinical assignments using human and other resources.
    • Participates in identifying cost effective measures and Kaizen initiatives for clinical care.
    • Active participation and contribution towards unit success through unit-based projects, shared governance council and task force meetings.
    • Demonstrates leadership (i.e. delegation, accountability, ownership, etc.) Analyzes the work to be done and plans for effective utilization of available resources.


    Nursing Research: Nurses perceive quality as the foundation of patient care delivery. Nurses participate in operations which promote, support and monitor and improve patient safety and outcomes. Nurses integrate evidence based practice and research into daily operations and provision of care.

    • Reads health care/patient care related research studies and, with assistance, participates in applying findings to practice.
    • Participates in the monthly collection of unit based quality data.


    Nursing as a Profession: Nurses role model and promote the image and professional status of nursing.

    • Clearly and efficiently communicates significant information from one level or location of care and/or from one caregiver to another.
    • Independently collaborates with peers and, with assistance, with members of the health care team.
    • Seeks leadership's guidance in conflict resolution.
    • Demonstrates participation in continuing education credits.


    Nursing Education: Nurses promote and foster the art and science of nursing through educational efforts.

    • Independently develops, evaluates and revises individualized, population-specific teaching plans for all patients.
    • Provides patient and family education that meets the diverse needs of the patient population.
    • Participates in the education of peers
    • The Cardiac and Pulmonary Rehabilitation RN will effectively manage patients with pulmonary disease and post cardiac surgery/intervention. The Rehab RN provides direct patient care in alignment with the standards established by the AACVPR.
    • Tasks and responsibilities include telemetry monitoring and interpretation, establishing patient goals and outcomes, and providing progress reports and communication via the patient monitoring system.


    Qualifications

    Educational / Minimum Requirements:

    • Current CT RN license
    • Basic Life Support (BLS)
    • Advanced Cardiac Life Support (ACLS)


    State/Federal Mandated Licensure or Certification Requirements:

    • Current State of Connecticut licensure as a Registered Nurse.


    Bristol Hospital Mandated Educational Requirements:

    General orientation at time of hire. Fire/Safety/Infection Control annually. Health Care Provider CPR from American Heart Association. Successfully complete all training programs as determined by the Professional Development department. Other unit based competencies/skills as required.

    Special Requirements:

    Successfully complete unit based competency package, plus unit specific items. Strong team skills.

    Physical Requirements:

    Physical agility with repetitive occurrence including but not limited to; walking, running, bending, stooping, kneeling, crouching, crawling, balancing, reaching, standing, sitting, and twisting. Looking at a computer monitor. Vision acuity correctable to normal. Hearing acuity correctable to normal. Acute sense of smell for normal perception. Ability to verbalize so that the average person can comprehend.

    Work Environment:

    Regular exposure to patient elements, occasional stress.

    A Brief History
    The Hayward Area Recreation and Park District, known locally as “H.A.R.D.," is an independent special use district created to provide park and recreation services for over 309,000 residents. Our boundaries encompass a 100 square-mile area, which includes the City of Hayward and the unincorporated communities of Castro Valley, San Lorenzo, Ashland, Cherryland, and Fairview.

    In 1944, representatives of the City of Hayward, various school districts, and local civic organizations developed a proposal for establishment of the Hayward Area Recreation and Parkway District. When an initiative was placed on the November 7th ballot, the residents voted overwhelmingly for the creation of the District, which was officially formed on December 11, 1944. In 1958, the District changed its name to the Hayward Area Recreation and Park District, and has become the largest recreation district in the State of California.

    Since its creation, the District has provided residents with many beautiful facilities and parks, many of which have received national and state recognition for their design, innovation and beauty, as well as hundreds of educational and recreational classes and programs.
