Job Description

Serve as resource about all topics related to entry into DD waiver and GR funded service. Work in partnership with staff within the Regional Office and TCM entities to provide resources, information, and support for individuals/families.

Maintain an information/resource library focused on “serving the all”. Identify and maintain a list of resources and organizations, including natural supports, generic, and disability specific, that provide an array of goods and services specific to support and/or caregiving for consumers. Identify and collaborate with existing family or parent organizations in order to connect families to local support groups in the area.

Serve as subject matter expert on one of four areas of specialty: Autism, MOCDDS, Medicaid eligibility, or Coordination with UMKC.

Provide content for trainings related to Division and other services, targeted to families but available for others. Coordinate efforts to educate stakeholders on community and natural supports, as well as services and options available through the Division and other sources.

Identify individuals who are appropriate for referral to the TCM’s, and manage supervision handoff efficiently.

Act as an ombudsman for Regional Offices - able to assist non-TCM individuals/families with a wide array of issues.

Serve as the local/regional contact between DD and the autism PAC’s.

Maintain a feedback/tracking system for data analysis of family and individual issues.

Maintain distribution lists for mailings, emails etc.

Review and provide technical assistance/ feedback regarding guardianship packets that will be filed by the Office of General Counsel.

Support families in writing the ISP’s for justification and request of the Missouri Children with Developmental Disabilities Waiver.

One or more years of experience as a Developmental Disabilities Community Worker II and/or as a Case Manager II (Developmental Disabilities) with the Missouri Uniform Classification and Pay System.

A Bachelor's degree from an accredited college or university with a minimum of 24 earned semester hours or 36 earned quarter hours in one or a combination of the following: Elementary or Secondary Education, Special Education, Early Childhood Education, Psychology, Social Work, Sociology, Counseling, Recreation (including specialty areas such as Art, Dance, Music, or Physical Education), Speech-Language Pathology or Audiology, Occupational Therapy, Physical Therapy, Nursing, or other specialties in the field of human services; and

Four or more years of professional experience as a Registered Nurse, or in social work, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, or a closely related area, including two or more years of experience working with persons with developmental disabilities.
