(Job # 536061 ) Registered Nurse II (10/12), Salary Range: $3,641.00 - 7,024.00 monthly. Anticipated Hiring Range: $3,641.00 - $4,900.00 monthly. Appointments are typically made at the beginning of the salary range. Salary is commensurate with qualifications and experience. This is a full-time, benefited, exempt, 10/12 pay plan (work 10 months of the year with pay and benefits over 12 months) permanent position with a one-year probationary period in Student Health & Wellbeing Services. Position Summary: Under regular supervision, the RN II is responsible for providing high quality medical triage and assessment, reassessment, and follow up of patients, managing patient flow, and for performing a variety of nursing care and administrative duties in the acute care clinic of the Student Health Center. Key Responsibilities: 40% - Intake and Management of Patient Flow: Identify, assess, and analyze problems in the clinic with team members to identify staffing needs in order to direct patient flow efficiently. This includes communication with supervisor and others regarding daily support services, need for assistance, and change of assignments in the case of unplanned absences. Determine appropriate patient centered care including assessment, triage, and monitoring while working within standard policies, procedures and protocols. Provide clinicians with written documentation inclusive of chief complaint and nursing observations / diagnosis based on patient's physical condition, vital signs, and interpretation of information obtained during patient intake; as well as patient acuity (level of clinical urgency). Focus on patient education, self-care and preventive health care measures within acute care and mental health treatment settings. Daily problem solving to address individual patient condition (medical, psychological, and financial situation) within scope of RN practice. Maintain working knowledge of patient resources and availability of programs. Initiate review of daily clinic flow (reviews charts day prior) and follows up on complicated patient situations (physical and mental health) among RN staff and clinical team as appropriate. In concert with medical providers, initiates emergency care, medications, labs, x-rays. diagnostic tests, medical/mental health intake and referrals according to protocols Administer first aid to patients, faculty, staff, and visitors as appropriate. Maintain thorough working knowledge and safe administration of SHC clinic medications including emergency drugs, IV fluids and naloxone (Narcan). Independently counsel patients in an objective nonjudgmental manner related to Family Planning, Medication Abortion, Sexual and Reproductive health, and LGBTQ care. Independently determine infection risk at Triage, direct staff to room patients within pandemic guidelines, has working knowledge of available daily patient and staff resources to streamline patient care. Serve as triage nurse making first contact with, assessing, and directing flow of patients and independently answering questions from reception, MAS, RNS, and Providers. This includes assuring that patients are assessed, interviewed, checked in, and safely discharged following appointments; that appointments are well monitored and tracked in the EMR. 35% - Clinic Duties: Triage patients based on clinical findings; begin treatment and work-up as appropriate. Administer medications and collaborate with medical team on the effectiveness of oral/medical injections/inhalation treatments. In concert with medical providers. responds to emergencies, including allergic reactions, fainting, vomiting, respiratory/cardiac arrest. Room patients; take vital signs; record clinical assessment; provide treatments as appropriate (such as, wound care, dressing changes, removal of stitches according to protocol, splinting, eye or ear irrigation, throat swabs, small volume nebulizer treatments, EKGs), assist with minor surgical procedures, perform IV insertions, fluids/medications as ordered; monitor, maintain and assess effectiveness of therapy in collaboration with medical team. Provide prefab orthopedic supplies, crutches, custom made splints and give instructions on use written and verbal. Manage Travel and Positive TB Consults in concert with medical provider. Independently manage reproductive needs utilizing RN protocols (work collaboratively with patient to meet their need for birth control and STI protection). Provide follow-up care within RN scope of practice. Provide education and counseling to pregnant patients, including pregnancy options as well as resources and referrals. Independently interview patients in regard to STI exposure, and utilize RN protocols to order labs, offer treatment, and independently provide follow-up care per protocol. Initiate specific lab Orders and give results per protocol (such as UA dip and urine pregnancy test: self-collected wet mount and Chlamydia Gonorrhea urine, throat, vagina and rectal: HIV, RPR, & Hep B blood tests: Quantiferon Gold; titers for MMR and Varicella). Coordination, collaboration, and provision of student immunization monitoring and TB surveillance. Close clinic at end of the day, making sure that patients have left and if not, staying until they have left. Responsible for complete legal records documenting nursing clinical care. Consult and communicate with nursing supervisor or medical director as appropriate. Ensure availability of medical supplies in exam rooms; notify purchaser of low stock. Ensure that medications and clinical treatment trays are current. Ensure backpack medications and emergency supply areas are stocked and within expiration period. Monitor security of clinic medications and ensure handling is done according to protocols determined by the Pharmacist-in-Charge. 20% - Administrative / Other: Manage programs, such as immunizations & TB screening and assure adherence to regulations. Monitor clinical processes required for compliance with accreditation standards and state legislative, and CO regulations. Participate in developing & implementing policies and procedures, particularly in realms of expertise (e.g. family planning, medication-based abortions). Assist Nursing Supervisor in training new staff and assist with preceptorship students (including being shadowed). Establish and maintain effective working relationships with SHWS colleagues and external medical services (including Planned Parenthood, Public Health, and local hospitals). California Children's Services (CCS) California Children’s Services (CCS) is a statewide program that provides case management and care coordination for clients' birth to 21 who have chronic, disabling and/or life threatening conditions. There are medical, financial and residential eligibility requirements. Under the direction of the CCS Nurse Manager, the Registered Nurse IV (RN IV), the Registered Nurse II (RN II) performs medical utilization review and comprehensive case management for a case load of children who have CCS medically eligible conditions. The RN II works in multi-disciplinary teams to determine medical eligibility and provides medical care coordination from the point of intake and initiation of the treatment care plan as part of the CCS interdisciplinary case management team. Nurses also ensure that clients are receiving necessary services with the most appropriate provider. The RN II is expected to have knowledge of: principles, methods and procedures for utilization review and relating to acute hospitalizations, home care, medical and nursing services and other levels of institutional care, CCS and Medi-Cal regulations; health care delivery system with respect to medical services available to the child and family; and family adaptation to handicapping conditions involving physical and psycho-social needs. Tuberculosis (TB) Control Section Registered Nurse IIs in the TB Control Section work under the supervision of the TB Nurse Manager (RN IV) and under the guidance of an RN III to provide case management to a caseload of clients with active and suspected active TB disease; to conduct monthly visits to the family, investigate contacts and assure access to care; oversee direct observation therapy (DOT) as needed, and to perform other duties as assigned. Nurse-Family Partnership (NFP) Nurse-Family Partnership is a home visiting program that partners nurses with low income first-time moms in Alameda County, in order to give babies the best start in life. Case management services begin in pregnancy and continue until the child reaches two years old. During that time, parents are provided with health education, support, and child development information so that families can create better lives for their child and themselves. This position will be situated within Public Health and includes nursing care management with prenatal and postpartum clients and their children up to the age of two. Nurses in this program receive intensive training to administer this evidence based home visiting program whose goal is to improve birth outcomes, increase child development and increase families' self-sufficiency. Nurse-Family Partnership nurses establish relationships with young, at-risk mothers during home visits and provide guidance for the emotional, social, and physical challenges first-time moms face as they prepare to become parents. NFP nurses work with culturally diverse families in communities with broad-based socio-inequity. We serve a high population of Oakland clients. Foster Care Assessment Center The Assessment Center (the AC) is a non-residential, short-term shelter child-friendly environment that operates 24 hours per day. The AC is staffed and managed by West Coast Children's Clinic, Inc., a community-based organization under contract to the Alameda County Social Services Agency. The AC has the capacity to receive and care for all ages of children and youth newborn to 21 years of age (Non-Minor Dependents NMDs).The purpose of the AC is to provide a supportive, child-friendly place where children can be looked after safely while more thoughtful placements are researched, including assessment of relative placement options, allow qualified staff to provide timely crisis intervention services to lessen the trauma of removal and initiate basic physical and mental health screenings, appropriate linkages and referrals. Under the supervision of the RN IV, the PHN II at the AC serves as an interdisciplinary team member of the Department of Children and Family Services (DCFS) and is responsible for implementing the daily nursing operations at the AC, which includes conducting nursing assessments on children received at the AC; communicating the need for timely follow-up of abnormal health assessment findings and/or acute health findings to placement providers, medical providers, child welfare staff, childcare staff, and PHNs in the Health Care Program for Children in Foster Care; facilitating the coordination of care between the AC, DCFS, Public Health Department, emergency foster homes, health care providers, schools, and any other relevant agencies invested in the total care of vulnerable children; providing onsite skilled, professional nursing consultation to the child welfare staff, including the Child Welfare Workers and other relevant personnel, child care staff, and biological and/or foster family; collaborating with child welfare staff to develop and maintain a systematic process for obtaining the basic medical history of the child at the time of removal from the caregiver’s home; gathering and interpreting information obtained from birth records, medical/dental records, immunization records, lab and radiological reports; documenting relevant information in the CWS/CMS database; researching medical insurance coverage for children to assist with urgent medical needs; training childcare providers on method of administering a brief health checklist for children brought to the AC; taking the lead to address the identified health care needs of each child/youth/non-minor dependent (NMD) admitted to the AC, including coordination of needed medical appointments, follow-up, and medication refills; administering, supervising and monitoring all medications prescribed; performing prescribed treatments. IHSS Program The IHSS Program is a home visiting program across all 58 counties in California. The Alameda County Public Health Nurse primary role is to conduct an assessment/reassessment of clients that are at risk for out of home placement and to prevent premature institutionalization and/or nursing home placement. The program serves eligible individual across the lifespan who are aged, blind, and disabled to remain safely in their own homes. The public health nurse carries a caseload focused exclusively on IHSS recipients identified as having paramedical service needs for example GT feedings/Tracheostomy/diabetes management/home dialysis. The PHN conducts comprehensive in-home assessment and reassessment to address the medical, nursing, and psychosocial aspects of the client’s health. In addition, the PHN provides health education and management of chronic medical conditions. The PHN is expected to have knowledge of the health delivery system, applying the nursing process, comfortable conducting a thorough assessment, and communicate effectively with clients. Case management services begin in pregnancy and continue until the child reaches two years old. During that time, parents are provided with health education, support, and child development information so that families can create better lives for their child and themselves. This position will be situated within Public Health and includes nursing care management with prenatal and postpartum clients and their children up to the age of two. Nurses in this program receive intensive training to administer this evidence based home visiting program whose goal is to improve birth outcomes, increase child development and increase families' self-sufficiency. Nurse-Family Partnership nurses establish relationships with young, at-risk mothers during home visits and provide guidance for the emotional, social, and physical challenges first-time moms face as they prepare to become parents. Foster Care Assessment Center The Assessment Center (the AC) is a non-residential, short-term shelter child-friendly environment that operates 24 hours per day. The AC is staffed and managed by West Coast Children's Clinic, Inc., a community-based organization under contract to the Alameda County Social Services Agency. The AC has the capacity to receive and care for all ages of children and youth newborn to 21 years of age (Non-Minor Dependents NMDs).The purpose of the AC is to provide a supportive, child-friendly place where children can be looked after safely while more thoughtful placements are researched, including assessment of relative placement options, allow qualified staff to provide timely crisis intervention services to lessen the trauma of removal and initiate basic physical and mental health screenings, appropriate linkages and referrals. IHSS Program The IHSS Program is a home visiting program across all 58 counties in California. The Alameda County Public Health Nurse primary role is to conduct an assessment/reassessment of clients that are at risk for out of home placement and to prevent premature institutionalization and/or nursing home placement. The program serves eligible individual across the lifespan who are aged, blind, and disabled to remain safely in their own homes. The public health nurse carries a caseload focused exclusively on IHSS recipients identified as having paramedical service needs for example GT feedings/Tracheostomy/diabetes management/home dialysis. The PHN conducts comprehensive in-home assessment and reassessment to address the medical, nursing, and psychosocial aspects of the client’s health. Selecting, obtaining or administering the anesthetics, adjuvant drugs, accessory drugs, fluids and blood products necessary to manage the anesthetic. Performing all aspects of airway management, including fiberoptic intubation. Performing and managing regional anesthetic techniques including, but not limited to, subarachnoid, epidural and caudal blocks; plexus, major and peripheral nerve blocks; intravenous regional anesthesia; transtracheal, topical and local infiltration blocks; intracapsular, intercostal and ocular blocks. Providing appropriate invasive and noninvasive monitoring modalities using current standards and techniques. Recognizing abnormal patient response during anesthesia; selecting and implementing corrective action and requesting consultation whenever necessary. Evaluating patient response during emergence from anesthesia and instituting pharmacological or supportive treatment to insure patient stability during transfer. Postanesthesia Care Providing postanesthesia follow-up and evaluation of the patient's response to the anesthesia and surgical experience; taking appropriate corrective actions and requesting consultation when indicated. Initiating, ordering, and administering respiratory support to ensure adequate ventilation and oxygenation in the postanesthesia period. Initiating, ordering, and administering pharmacological or fluid support of the cardiovascular system during the postanesthesia period to prevent morbidity and mortality. Initiating, ordering, and administering acute postanesthesia pain management techniques. Discharging patients from a postanesthesia care unit (PACU) according to facility policy. Clinical Support Functions Inserting peripheral and central intravenous catheters. Inserting pulmonary artery catheters. Inserting arterial catheters and performing arterial puncture to obtain arterial blood samples. Managing emergency situations, including initiating or participating in cardiopulmonary resuscitation. Providing consultation and implementation of respiratory and ventilatory care. Management of interventional pain therapy using drugs, regional anesthetic techniques, or other accepted pain-relief modalities. Selecting, obtaining, ordering and/or administering medications or treatments related to the care of the patient, using consultation when appropriate. Accepting additional responsibilities which are within the expertise of the individual CRNA and appropriate to the practice setting. Requirements SELECTION REQUIREMENTS: To qualify, candidates must meet all of the following requirements: 1) State licensure by the California Board of Registered Nursing as a Registered Nurse. 2) State certification by the California Board of Registered Nursing as a Nurse Anesthetist. 3) Graduation from a program of nurse anesthesia education accredited by the Council on Accreditation of Nurse Anesthesia Educational Programs of the American Association of Nurse Anesthetists.* 4) Certification or Recertification by the National Board of Certification and Recertification for Nurse Anesthetists (NBCRNA). LICENSE: A license to practice as a Registered Nurse issued by the California Board of Registered Nursing. PHYSICAL CLASS: Physical Class II - Light: Light physical effort which may include occasional light lifting to a 10 pound limit, and some bending, stooping or squatting. Considerable walking may be involved. DESIRABLE QUALIFICATIONS: Experience as a Certified Registered Nurse Anesthetist (CRNA) Applicants claiming experience as a Certified Registered Nurse Anesthetist in a state other than California must include their certification number on the application for verification at the time of filing. Experience claimed without a valid certification number will not be considered.
