    Responsibilities:
    • Full Scope Primary Care:

      • Provide a full range of primary care services, including diagnosis, treatment, coordination of care, preventive care, and health maintenance.
    • Clinical Expertise:

      • Review patient history and conduct thorough physical examinations.
      • Evaluate, diagnose, and provide appropriate treatment, referring patients to specialists as needed.
      • Utilize quality clinical diagnostic skills in patient care, reviewing test results and recommending suitable management plans, including preventive health recommendations.
    • Prescriptive Authority:

      • Prescribe medications and formulate ongoing treatment and disease management plans.
    • Relationship-Centered Care:

      • Provide relationship-centered care, considering the patients psychosocial and physical needs.
    • Documentation and Communication:

      • Collect and record patient information, maintaining complete and accurate medical records.
      • Communicate effectively with the clinical team to ensure the total delivery of quality care.
    • Educational Engagement:

      • Participate in medical staff educational programs and meetings.
    Qualifications:
    • Education and Training:

      • Doctor of Medicine (MD), Doctor of Osteopathy (DO), or foreign equivalent.
      • Completion of a three-year post-graduate medical training from a U.S. accredited residency program in Family Medicine or Internal Medicine.
    • Experience:

      • Seeking both experienced and newly trained providers.
    • Licensure and Certifications:

      • Board Eligible/Board Certified in Family or Internal Medicine.
      • Current, unrestricted New York State license.
      • Active Drug Enforcement Agency (DEA) registration or eligibility.
      • Active Medicare/Medicaid Enrollment or eligibility.
      • BLS or ACLS required.
    • Communication and Technology Skills:

      • Strong communication skills, both oral and written.
      • Comfortable with Electronic Medical Record (EMR) systems and Microsoft Office.
