Yale New Haven Health - - Responsibilities: Coordinate the collection, documentation and reporting of information necessary to ensure the preparation of official birth certificates by local governmental Departments of Vital Statistics.; Prepare, distribute and collect birth and/or paternity documents from Maternity Services daily and ensure completeness.; Coordinate acquisition of social security numbers for neonates and assist patients in information required for neonatal SSN.; Coordinate compliance with immunization guidelines and office of legitimations requirements.; Coordinate preparation of data forwarded to Yale-New Haven Hospital departments.
