The Public Health Bureau is seeking to fill four full time Behavioral Health Aide positions in the Enhanced Care Management (ECM) Program. The ECM Program serves clients who are experiencing homelessness, have chronic medical conditions, are high utilizers of the emergency department and/or hospitalizations, and/or have severe mental health diagnosis. The BHA will focus on supporting the social and behavioral goals of the clients, provide coaching and education around goals, maintain regular contact with providers and community supports, and assist connect clients to social support resources. The BHA may also transport clients to and from appointments as needed. They will work within an interdisciplinary team of Public Health Nurses, Public Health Licensed Vocational Nurses, Social Workers, and Community Services Aides. The Behavioral Health Bureau serves the Medi-Cal population of Monterey County including, adults and young adults with serious and persistent mental illness and children, youth, and families with severe mental health difficulties. The Behavioral Health Aides are an integral part of our multidisciplinary teams. Currently, the Behavioral Health Bureau is seeking to fill four full time Behavioral Health Aide positions within the Adult System of Care (ASOC), Children's System of Care (CSOC), Forensic Team, and South County Access programs. ASOC: The Adult System of Care programs in Salinas and Marinaprovide a wide range of clinical care services to adults with serious mental illness. These programs are oriented towards a strength-based, wellness-based, whole person approach that facilitates each person's progress towards their recovery journey. Staff work within a multi-disciplinary team consisting of Psychiatrists, Nurse Practitioners, Nurses, Clinicians, Social Workers, and Behavioral Health Aides.
