Job Summary: Under the general direction of a medical provider and the Executive Director, the Registered Nurse III (RN III) is responsible for the development and implementation of nursing standards, policies, and procedures for the Student Health Center (SHC), and oversees and directs the nursing services for one or more specialty areas for the SHC. Schedules and assigns Nursing work to ensure adequate coverage, training and orienting employees, and providing input to employee selection and performance evaluations; identify professional growth and development needs for assigned staff and communicate needs to supervisor; develop and implement in-service training. Coordinates daily nursing operations and ensure appropriate staffing levels to meet clinic needs. May assist in developing and monitoring the nursing budget. Accreditation: developing nursing standards, policies, procedures, protocols and standing orders in collaboration with other team members; Maintenance of logs and facility compliance with accreditation standards. Addresses patient concerns, refers to administrator when warranted. Clinic Coordination/Special Projects Serve as liaison between nursing staff, providers, front office and other clinical team members to ensure effective communication in support of patient care; review schedules to anticipate staffing shortages or other challenges in order to avert them or plan ahead; lead weekly team meeting. Oversees the vaccine program, including but not limited to, overseeing proper storage/handling/administration and staff-related training • Leads and coordinates specialty clinics (e.g., flu vaccine), health fairs and student orientations. Assists in the implementation and operationalization of new services and the ongoing improvement of current ones. Leads Nursing Peer Review and Quality Improvement and collaborates with other team members on Emergency Preparedness, Infection Control & Prevention and Accreditation Assists in monitoring compliance with infection control and prevention policies and procedures. Participates in clinic and staff conferences and other presentations when appropriate. Nursing Triage/Back up Nursing Coverage Serve as Nurse Lead for triage questions. Serve as back up triage RNs as needed. Utilizing established triage protocols, functions as the telephone/walk-in triage nurse by assessing patients’ signs and symptoms to determine appropriate disposition (schedules same-day, urgent or future appointments using electronic health records system) and/or treatment. Utilizing standing orders provides first aid to student, staff, faculty and campus visitors. Per provider request, will contact patients to give information regarding referrals, prescriptions, recheck appointments. Provide sexual health education, counseling and care, including administration of medication abortion services. May serve as backup in preparing patients for telehealth and medical examinations and treatment • May perform routine laboratory testing (e.g., urinalysis); may obtain lab samples obtained my medical providers for pick up by the lab. Assists with medical examinations and procedures as requested by medical providers. Provides education counseling on a wide variety of health topics (e.g., STIs, contraception) •Responsible for following Tuberculosis Screening Guidelines - administering PPDs, ordering chest x-rays, ordering laboratory testing and facilitate follow up appointments for patients, when needed. With prior approval from individual providers, communicate normal lab results to students; for results outside of normal limits, facilitate appointment scheduling with a provider. Under standing orders will evaluate requests for HCG including patient assessment, testing, pregnancy options counseling, and appropriate referrals. Engage in professional development activities to remain up-to-date and to meet ongoing licensure requirements; completes all department and university required trainings as assigned. Minimum Qualifications: Required Education and Experience Three years of progressively responsible professional nursing experience which has provided evidence of the ability to perform administrative and lead responsibilities; or equivalent combination of education and experience which also provides the required knowledge and (including the ability to work independently in specialized nursing activities) and support the overall nursing program within the Student Health Center Possess and maintain a valid license as a Registered Nurse in the State of California and certification in CPR/AED. Required Qualifications Possess a working knowledge of effective supervisory methods and techniques; more comprehensive understanding of nursing programs and specialties; possess broad and specialized nursing knowledge that supports the performance of specialized duties in such areas as, but not limited to, triage and assessment, patient counseling, and staffing specialized nursing clinics; broad knowledge of clinic operations and the development and implementation of a nursing program, including procedures and protocols; thorough knowledge of professional nursing principles, methods and procedures; anatomy and physiology applicable to assigned duties; medical supplies and equipment used in nursing practice; and the uses and effect of medications and narcotics; working knowledge of applicable state and federal laws and regulations pertaining to registered nursing and the scope of practice limitations of clinic support staff. Correctional Health Services provides health care to adult inmates at the San Joaquin County jail facility, located at 7000 Michael N. Canlis Blvd, French Camp, CA 95231. The care is mandated by Title 15, Division 1, Sub-Chapter IV of the California Regulatory Code. To work in the County jail facility, applicants must pass a rigorous security clearance which is a condition of employment. More information about the San Joaquin County jail facility and the mandatory security clearance is contained in the supplemental application which must be completed and submitted with the standard County employment application. * S upplemental Pay: Registered Nurses employed at Correctional Health Services shall receive a 5% Nurse Specialty Pay per C.N.A. MOU. Shift: Correctional Health Services shifts are 12 hours per shift. Rotating Weekends: Candidates must be available to work every other weekend and be available on an as needed basis. Correctional health care (CHC) nurses are in charge of the medical care of inmates. The nurse is responsible for completing health assessments, conducting triage, accepting or refusing patients in booking, making appropriate medical, dental, and mental health referrals. CHC nurses work in multiple areas including a busy medical unit with the potential of 35 patients to one RN. Assessment and documentation skills are essential in a jail setting due to the legal responsibility and potential for lawsuit. The nurse is responsible for overseeing the work of LVN’s, psychiatric technicians, medical and dental assistants. The nurse must also have excellent skills in venipuncture, IV infusion including PICC line care, drug calculations, and medication administration. When needed, the RN provides emergency medical care. Duties are including but not limited to performing CPR, applying AED, infusing intravenous medications, wound care, and initiating standardized procedures. Casework may include home visits (to client's residence, hospital, board & care home, health clinic, day program or skilled nursing facility) to conduct an assessment, engagement of the client and client's informal and formal support system in accordance with the service plan of the APS Social Worker, which is designed to reduce or eliminate risk for the client. PHNs will provide health education to clients and their caregivers and/or family and will advocate on behalf of clients for access to required medical services. PHN Case Managers may recognize and coordinate care for varying stages of dementia and cognition; identifying and facilitating appropriate level of care, which could lead to out-of-home placement (such as skilled-nursing facilities, board and cares etc.); facilitating the enrollment process and care provider selection for In-Home Support Services (IHSS) or the selection of other care appropriate providers; and coordinating services, such as Meals-on-Wheels, Friendly Visitors, transportation services (i.e. Paratransit). Nurse Family Partnership Nurse-Family Partnership is a home visiting program that partners nurses with low income first-time moms residing primarily in Oakland, in order to improve birth outcomes, increase self-sufficiency and improve child development. Case management services begin in pregnancy and continue until the child reaches two years old. During that time, families receive health education and assessments, child development information and screenings and help with goal setting so that they can create better lives for their child and themselves. Nursing Administration The Nursing Administration Office sits inside of the Public Health Nursing Division and is comprised of the Central Intake and Referral Unit, Education and Professional Development Unit, Public Health Nurse Recruitment and Retention Unit, Quality Improvement and Nurse Informatics Units, and Occupational Health Unit. Public Health Nurses (PHNs) as analysts, consultants, coordinators, educators, improvement specialists, and recruiters work as part of multi-disciplinary teams in the Nursing Administration Office to advance and strengthen the role, function, and development of PH Nursing services. Staff in the Nursing Administration Office collaborate with internal and external partners in the development of policy and professional nursing standards of care that support the health of Alameda County residents. Tuberculosis Control (TB) The Tuberculosis Control program staff includes public health nurses, community health outreach workers, physicians and clerical support staff working collectively toward the program's priorities and objectives to: Ensure early identification and reporting of all persons with TB and ensure timely completion of appropriate therapy; Ensure contacts to persons with infectious TB are promptly identified, examined, and if appropriate, complete treatment for latent TB infection (LTBI); Reduce occurrence of sentinel events (pediatric TB cases and TB deaths); and Identify persons (other than contacts) with LTBI at risk for progression to TB disease, and treatment of those persons with an effective drug regimen. TB staff also consults with physicians, schools, hospital nurses and others in the community on Tuberculosis. Additionally, they monitor the number of TB cases yearly and determine other steps that need to be taken to control Tuberculosis in Alameda County. IHSS Program The IHSS Program is a home visiting program across all 58 counties in California. The Alameda County Public Health Nurse primary role is to conduct an assessment/reassessment of clients that are at risk for out of home placement and to prevent premature institutionalization and/or nursing home placement. The program serves eligible individual across the lifespan who are aged, blind, and disabled to remain safely in their own homes. The public health nurse carries a caseload focused exclusively on IHSS recipients identified as having paramedical service needs for example GT feedings/Tracheostomy/diabetes management/home dialysis. The PHN conducts comprehensive in-home assessment and reassessment to address the medical, nursing, and psychosocial aspects of the client’s health. In addition, the PHN provides health education and management of chronic medical conditions. The PHN is expected to have knowledge of the health delivery system, applying the nursing process, comfortable conducting a thorough assessment, and communicate effectively with clients. ALAMEDA COUNTY PUBLIC HEALTH NURSES Public Health Nurses (PHNs) work as part of multidisciplinary teams and are embedded inside well-funded comprehensive programs to ensure that our diverse, low-income, multi-stressed individuals, families, and communities are served in holistic ways. PHNs work in multiple programs/units as case managers, community engagement specialists, consultants, coordinators, educators, health screeners, inspectors, investigators, and vaccinators, in the areas of Children’s Medical Services, Central Referral and Intake, Chronic Disease, Communicable Disease, Community Engagement, Immunization Assistance, Correctional Health, Maternal, Child and Adolescent Health, Nurse Education, Nurse Recruitment, Occupational Health, Older Adults and Public Health Emergency Preparedness and Response. The Public Health Department maintains a Memorandum of Understanding with the Alameda County Social Services Agency for the provision of PH Nursing services to clients in the Departments of Adult and Aging Services (Adult Protective Services, Public Conservator, Public Guardian, and In-Home Supportive Services) and Children and Family Services (Foster Care Assessment Center and the Medically Fragile and Services to Enhance Early Development Units). Descriptions of some of these program/units are specifically highlighted below: PROGRAM OVERVIEW Adult Protective Services (APS) and Foster Care PHNs provide nursing case management services to clients with complicated chronic medical conditions. These conditions often render the client vulnerable to ongoing abuse and neglect and at risk for additional medical emergencies, further traumatizing clients and creating cycles of recidivism. Casework may include home visits (to client's residence, hospital, board & care home, health clinic, day program or skilled nursing facility) to conduct an assessment, engagement of the client and client's informal and formal support system in accordance with the service plan of the APS Social Worker, which is designed to reduce or eliminate risk for the client. PHNs will provide health education to clients and their caregivers and/or family and will advocate on behalf of clients for access to required medical services. PHN Case Managers may recognize and coordinate care for varying stages of dementia and cognition; identifying and facilitating appropriate level of care, which could lead to out-of-home placement (such as skilled-nursing facilities, board and cares etc.); facilitating the enrollment process and care provider selection for In-Home Support Services (IHSS) or the selection of other care appropriate providers; and coordinating services, such as Meals-on-Wheels, Friendly Visitors, transportation services (i.e. Paratransit). Nurse Family Partnership Nurse-Family Partnership is a home visiting program that partners nurses with low income first-time moms residing primarily in Oakland, in order to improve birth outcomes, increase self-sufficiency and improve child development. Case management services begin in pregnancy and continue until the child reaches two years old. During that time, families receive health education and assessments, child development information and screenings and help with goal setting so that they can create better lives for their child and themselves. Nursing Administration The Nursing Administration Office sits inside of the Public Health Nursing Division and is comprised of the Central Intake and Referral Unit, Education and Professional Development Unit, Public Health Nurse Recruitment and Retention Unit, Quality Improvement and Nurse Informatics Units, and Occupational Health Unit. Public Health Nurses (PHNs) as analysts, consultants, coordinators, educators, improvement specialists, and recruiters work as part of multi-disciplinary teams in the Nursing Administration Office to advance and strengthen the role, function, and development of PH Nursing services. Staff in the Nursing Administration Office collaborate with internal and external partners in the development of policy and professional nursing standards of care that support the health of Alameda County residents. Tuberculosis Control (TB) The Tuberculosis Control program staff includes public health nurses, community health outreach workers, physicians and clerical support staff working collectively toward the program's priorities and objectives to: Ensure early identification and reporting of all persons with TB and ensure timely completion of appropriate therapy; Ensure contacts to persons with infectious TB are promptly identified, examined, and if appropriate, complete treatment for latent TB infection (LTBI); Reduce occurrence of sentinel events (pediatric TB cases and TB deaths); and Identify persons (other than contacts) with LTBI at risk for progression to TB disease, and treatment of those persons with an effective drug regimen. TB staff also consults with physicians, schools, hospital nurses and others in the community on Tuberculosis. Additionally, they monitor the number of TB cases yearly and determine other steps that need to be taken to control Tuberculosis in Alameda County. IHSS Program The IHSS Program is a home visiting program across all 58 counties in California. The Alameda County Public Health Nurse primary role is to conduct an assessment/reassessment of clients that are at risk for out of home placement and to prevent premature institutionalization and/or nursing home placement. The program serves eligible individual across the lifespan who are aged, blind, and disabled to remain safely in their own homes. The public health nurse carries a caseload focused exclusively on IHSS recipients identified as having paramedical service needs for example GT feedings/Tracheostomy/diabetes management/home dialysis. The PHN conducts comprehensive in-home assessment and reassessment to address the medical, nursing, and psychosocial aspects of the client’s health. In addition, the PHN provides health education and management of chronic medical conditions. The PHN is expected to have knowledge of the health delivery system, applying the nursing process, comfortable conducting a thorough assessment, and communicate effectively with clients. Under the supervision of the Supervising Public Health Nurse (RN IV); the RN II/PHN serves as part of a multidisciplinary team who are responsible for assessing clients’ ability to live safely in their home by evaluating the risk for institutionalization, monitoring client’s progress through any prescribed rehabilitative, and providing case management in coordination with other departments, agencies, and health providers.
